**What do you like best about Forma?**

I like how quickly I was able to get my claims approved and more importantly, it was a simple process to get my claim submitted. The options for each field were comprehensive. The support to get my login issues was fixed very quickly. Once I got some issues resolved with my company's HR, the integration process with my benefits provider/employer was very simple. I can't speak on the pricing because that is dictated by my employer and I have no part in it.

**What do you dislike about Forma?**

I don't like the login process. The "magic link" being the default way to log in is just an obstacle to the routine username and password, which I prefer. I don't want to have to check my email every time I log in and rely on clicking specific links.

**What problems is Forma solving and how is that benefiting you?**

Forma is solving the issue of how I get reimbursed for certain expenses and that is helping me use my benefits in a less hassle way.

**What do you like best about Forma?**

I like that it allows me to process claims and make qualifying purchases with any claims approved. It gets back to me very quickly if the claim is either approved or denied.

**What do you dislike about Forma?**

I do not like that you when you want to cash out earnings to a bank account in Forma it is kind of confusing for someone just now starting to use it. Basically you either have to spend the funds for the claim on one of the items in the store on Forma or you have to make additional purchases then cash out the earnings by that of a qualifying reimbursement. There should be a way users can streamline the earnings they have from claims directly to their own pay accounts.

**What problems is Forma solving and how is that benefiting you?**

It solves the problem of submitting a claim or to receive reimbursements from another department or various departments. It  helps to benefit me because I know that if I submit a  claim today it will either be approved or denied by the end of my work day which helps me to better balance my own finances, and schedule accordingly around the reviews.

**What do you like best about Forma?**

I like that Forma is very intuitive and easy to use. It's user-friendly and makes it straightforward to see where I can file reimbursements. There are very few misleading items, which I appreciate. When I first used Forma, it was part of our product suite, and I found it very easy to get started. My manager gave me a quick run-through, and by the next time I had to file a reimbursement, I was able to do so easily.   I also really like the chat support for this one. There are support team members that are ready to assist with queries and they really address your issue competently - not just redirecting you to a guide or another resource. They are also very patient with confirmations and follow-up questions.

**What do you dislike about Forma?**

I think the interface could be a bit clearer. I think that there could be less buttons to use. The interface could be a lot simpler. It's simple now, but it could be a lot more simple and easier to use.

**What problems is Forma solving and how is that benefiting you?**

I use Forma for my company, mainly for filing reimbursements like Internet bills and educational assistance.

**What do you like best about Forma?**

The customer service has been great — whenever I have needed a claim approved or had a question about whether or not something was included in my benefits, their agents quickly got back to me. They are also very flexible in approving various services to meet the needs of myself and my colleagues. Reaching out to customer support is also very easy.

**What do you dislike about Forma?**

I find it can be a little bit confusing and overwhelming to figure out what's covered and what isn't. I've had questions about how to code my claims.

**What problems is Forma solving and how is that benefiting you?**

It can be difficult to justify spending money on myself and my personal wellbeing/lifestyle. Having the LSA account with Forma, that is specifically designed to be spent on those things, allows me to do things like travel and get massages without the guilty of spending money from my paycheck that could otherwise be used for things like rent or groceries. I've used my Forma account to get massage/reiki, book a plane ticket for a trip with my best friends, and even buy a book I'd been wanting to read. The flexibility and customer support allows me to quickly and easily be reimbursed for these expenses.

**What do you like best about Forma?**

What I like most about Forma is the flexibility it gives employees while still keeping administration straightforward. The platform lets us offer a broad range of wellness and lifestyle benefits without introducing unnecessary complexity, and the user experience makes it easy for employees to understand their options and actually engage with their benefits. From an admin perspective, the visibility into the program and the support we receive make it much easier to manage everything day to day and to advocate for employees when questions come up.

**What do you dislike about Forma?**

One challenge I’ve experienced with Forma is the quick turnaround required for certain requests or changes. At times, this makes it difficult to fully evaluate the potential impacts or coordinate internally before implementation. While I do appreciate the team’s responsiveness, having a bit more lead time in some situations. It would help administrators better support employees and ensure a smoother overall execution.

**What do you like best about Forma?**

Forma is a great benefit at work.  The site offers examples of items allowed in policy, give you options, and even offers direct buy selections with discounts. While I chose to use the card and purchase direct from a retailer, there are great offers available. Redeeming is easy.  I uploaded the receipt from the website I purchased through and got timely updates. There was an issue with my receipt so I contacted support. They explained what was wrong and showed me how to update.  The rep kept me posted and checked back in to see if I was satisfied.

**What do you dislike about Forma?**

The process is a little clunky.  When using the card to buy from an approved vendor it seems weird to upload a receipt.

Forma removes a barrier to self care and makes me feel invested in by my company.

I use Forma for benefit reimbursement programs at our company, and I love how simple it is to use and upload receipts. I don't need to manually type in all the information from the receipt because it uses AI to pull in all the information and pre-populate my reimbursement, which is super helpful. It's easy to submit reimbursement requests and see all my pending or approved requests, and from what I remember, it was pretty easy to set up.

**What do you like best about Forma?**

I use Forma for commute reimbursement, and it reimburses me really quickly with approval coming within a day. I like that it's easy to use and very straightforward, and the policies related to reimbursement are easy to access. I also find that a human representative is very responsive and able to solve problems almost immediately. Forma is integrated into our Okta SSO, and it definitely speeds things up for reimbursing certain perks.

**What do you dislike about Forma?**

When there is an error or issue, the support bot is NOT helpful. But a human representative was very responsive and able to solve my problem almost immediately.

Forma reimburses me really quickly, with approvals within a day. It's easy to use, straightforward, and reimbursement policies are easy to access.

**What do you like best about Forma?**

Forma is our latest platform for expensing our Perks+ fitness and mental health benefits and by far it's my favorite. It is very easy to use and the response time from the representatives are very quick. I have had expenses approved on the same day. Furthermore, if they need more information to move forward with the approval they send detailed instructions.

**What do you dislike about Forma?**

It's a great system and very easy to use. The only area of improvement is providing more detailed sub-categories so you can easily select what you are trying to reimburse.

I love how easy it is to login to the system. With the magic link, I just have to type in my email and then, the system sends me an email where I can click. It works so smoothly on my phone, which is more than you can say about a lot of these types of systems.

**What do you dislike about Forma?**

There's not much I dislink, but perhaps you could improve the phone upload of receipts, with some AI assistance to fill out the required fields, if possible. Currently, I scan my receipts using Dropbox, load in the PDF and still have to fill out all the fields in the form manually.

**What problems is Forma solving and how is that benefiting you?**

It provides me with easy access to our company benefits. I didn't really use any other tools, and neither did I have any problems, but Forma really provides an easy way for me to access my benefits, with direct link to the expense policy so I can see what I can charge for.

It would be great if this gift card could be used at more merchants. There are times when I want to use it, but the stores I visit don’t accept it.

Forma solves the problem of managing multiple employee benefits in different places. Instead of keeping receipts and submitting manual reimbursements, I can use my allocated funds directly through their card or approved merchants. This saves me time and makes it much easier to understand how much balance I have left. It also gives me more flexibility to use my benefits the way that works best for me.

**What do you like best about Forma?**

I like that there are tons of options and especially so many categories that you can choose from to buy different items. You can get equipment, apps, programs like nutrition help, memberships, and all kinds of different things that will help boost your health in ways that I just wouldn't think about otherwise. This variety gives more impact on my health. It lets me not just get a bunch of weights for exercise, but also offers options like scales for tracking weight and apps to track sleep because that's also part of health. It really expands the possibilities of the different ways that I can improve my health.

**What do you dislike about Forma?**

I didn't really like how things were reimbursed or restricted. I felt like the reimbursement process was really clunky unless you are strictly buying from the Forma store, which is kinda limiting because you buy something thinking that you can get reimbursed for it with Forma, and then they reject it. That's kind of frustrating. I also had an experience where my company's contract/list of approved items specifically listed that you could buy clothing or other items for your dependents, but when I tried to get athletic clothing for my child, they rejected it. I still don't understand why when it was in our contract.

Forma helps me by setting aside a budget for my health needs, which I might not make otherwise, and provides a variety of options to improve my health.

Forma is very easy to use and has integrated AI that scans most of the information from bills when uploaded, making the process really convenient by automating most of it. I find it very user-friendly. Forma's customer support is also exceptional, always going above and beyond to help out the customers upon reaching out to them. Additionally, the initial setup was really easy, just a simple registration with email confirmation and I was ready to go.

There could be more clarity provided related to employer program policy as there is a lot of ambiguity in terms of what all is exactly covered under the employer's policy. Forma should work together with employers to figure this out and provide better, more specific examples.

**What do you like best about Forma?**

What I like most about Forma is the simplicity and transparency it brings to reimbursements. The interface is easy to understand even as a new user, and submitting claims doesn't feel complicated or time-consuming. I can clearly see available budgets, past claims, and approval status all in one place, which removes a lot of guesswork. Uploading a receipt is straightforward, and the structured approval flow feels less messy. We also have a mobile application that lets us upload receipts on the go, which is great for quickly uploading them. We don't have to maintain our own tracker or follow up repeatedly, reducing the risk of missing out on reimbursable transactions. Overall, using company benefits feels smooth and predictable instead of stressful. It also helps us forecast our plans based on the available budget.

One area where Forma could improve is around the clarity and speed of the approval process. At times, it is not clear why a submission is delayed or if additional information is needed, which can lead to waiting without much context. Submitting a claim could result in it being rejected, while for another user, it might get approved, suggesting an ambiguous area that involves guesswork about whether an expense will be covered or not. Sending an email to get confirmation is an option, but having some form of metric to check against would be better. Additionally, the mobile app is not enabled for all organizations, so some cannot utilize this benefit.

What I like best about Forma is how simple and intuitive it is to use. I can quickly see my balance, submit an expense, and move on without having to think too much about the process. **What do you like best about Forma?**

I like that Forma makes it very easy to submit my invoices and request claims. There are a lot of options available to claim different things, which has been very helpful for me. It allows me to work comfortably.

**What do you dislike about Forma?**

I don't see there is much customer service when you're having issues. There's a specific team you can engage through chat, but it's not like they are actually reviewing the claim. I would like to be able to speak to the people reviewing the claim because when I need to enter a description for a claim, I don't have much option to express myself and share context - It's just like a text box, which is limited. In some instances, I actually have to be very specific for what I'm claiming. I want to think of this feedback as a way to help the people reviewing claims.

**What do you like best about Forma?**

I like that Forma sent me a physical card to use, which I can use in stores. It's also really convenient that I can use the credits I have in my account. The setup was pretty simple; you just log in, see $300 in your account, and with a few clicks, a card arrives in the mail a week later. I also appreciate getting a text message every time I use it that tells me how much is left in my account and reminds me to submit a receipt. So, that's great.

**What do you dislike about Forma?**

I wish that if I'm using my credit card in store, I didn't have to snap a picture of the receipt if you have all the details on the receipt from me using the card. That doesn't make sense to me.

**What problems is Forma solving and how is that benefiting you?**

Forma doesn't solve any problems for me. It's really just a way to receive money and spend it.

I'm so grateful for Forma because it helps me a lot with my physical and mental health, allowing me to be at my best when I come to work. It's also financially beneficial, helping me get a gym membership I would have hesitated to get otherwise. I really enjoy learning new skills, like becoming a better swimmer, thanks to the access it provides. The convenience is excellent—having the physical card in my wallet makes it easy to purchase things on the spot. The submission process is super easy and submissions get approved quickly. The customer support is also amazing, resolving minor issues within 24 hours every time. The initial setup was really quick and easy, and the website is extremely self-explanatory.

I really appreciate that Forma's lifestyle spending account covers my lifestyle needs like meditation classes, physical fitness classes, body massages, and necessary products like humidifiers and air purifiers. I like that the chat representatives are real people who understand when I ask questions about specific products, and I can directly talk to someone to understand if a product is eligible, which I find helpful instead of going through the eligibility section on the website. Also, the initial setup was a cakewalk.

**What do you dislike about Forma?**

The eligibility section on the Forma website is not really helpful because it's very generic and limited. It seems that the products listed are restricted, and a lot of things aren't covered. For example, not all massagers are applicable, and while heaters can be bought, fans cannot. I feel they should do more research to expand the list of eligible products and consider customer feedback. It shouldn't be restricted to what's currently listed.

I think it could give you a better menu of items that you could add to your claims, or more options. And, also, something about subscriptions. Like, for example, when you have a recurring claim that you wanna do, you have to do it every single month.

There are some additional merchant supports which were introduced as part of the benefits offerings like Forte music lessons,Sleep Cycle & Fitbod, Babbel Language learning etc. Many merchants work directly from your forma wallet and you don't have to claim reimbursements separately. The list keeps growing as they're adding more merchants and products.

The multisports facility is still not added as an service offering. I hope it gets added in the near future.

Forma has been solving the problem of managing the UChoose Program of UKG where employees are provided with certain amount of monetary benefits every quarter which they can spend on a vast range of defined categories determined by UKG.
