I use Survio for collecting yearly feedback from employees. It allows me to make forms with questions and analyze the responses. It's the questions form that collects feedback for me. They automatically take the money from my card, and I can't stop them. I've talked to support, and they tell me they do not offer to downgrade the tier, but they do it on their webpage, which is wildly misleading.
