"A Robust Platform for Patient Data, but Implementation is Heavy"

What do you like best about Agentforce Health (formerly Salesforce Health Cloud)?

The "Patient 360" view is the strongest feature. As a developer, I appreciate that the Care Observations data model (based on FHIR standards) is ready out-of-the-box. We use it to track patient vitals—like Blood Glucose levels and HbA1c—directly on the timeline. It means clinicians don't have to switch back and forth between the EMR and Salesforce to see a patient's progress. Review collected by and hosted on G2.com.

What do you dislike about Agentforce Health (formerly Salesforce Health Cloud)?

The initial configuration is extremely complex. Mapping the clinical data from an external EHR system into the Health Cloud object model requires a lot of effort and middleware (like MuleSoft). Also, the standard "Patient Timeline" component can be slow to load if a patient has thousands of historical records, sometimes frustrating the doctors. Review collected by and hosted on G2.com.

What problems is Agentforce Health (formerly Salesforce Health Cloud) solving and how is that benefiting you?

It solves the problem of "data silos" in healthcare. Previously, our care coordinators had to check three different systems to see if a patient was adhering to their diabetes care plan. Now, we bring that data into the Health Cloud Console. It allows us to trigger automatic tasks—like a follow-up call—if a patient's reported glucose level goes above a certain threshold, which improves our response time significantly. What do you like best about Agentforce Health (formerly Salesforce Health Cloud)?

This includes the majority of the industry-standard templates required for the whole Health industry, such as EHR, Accounts, and Medications. Household view, in which all members are linked together based on the member plan, with primary and secondary roles. Simple methods for connecting clinicians, suppliers, and their networks with member plans. Because it is designed on top of the service cloud, it has excellent interoperability with CTI platforms. What do you dislike about Agentforce Health (formerly Salesforce Health Cloud)?

It can feel like there is almost too much information to manage at times, but this is due to their being more capability accessible than what we are using. Runs slowly sometimes, but not frequently. Care coordination has improved across departments. It is simple to observe and understand where a patient has been seen or where he or she may benefit from more care.
