We utilize their platform to send our new patient paperwork and other forms to patients. It has been a wonderfully easy platform to use and learn. Our patients no longer tell us they have troubles opening documents or that they are unable to complete our new patient paperwork. We love how easy it is use and how easy it is for our patients! Highly reccommend trying FormDr if you send things electronically! **What do you dislike about FormDr?** The ONLY thing we have found to be downside (but really not) is that you are only able to search by patient name for records. We are still able to pull up patient data by last name and/or first name! **What problems is FormDr solving and how is that benefiting you?** We utilize FormDr to send all our new patient paperwork, some questionnaires and other forms. It is so easy to use and even easier for patients to access their forms electronically. We love that the patients can complete on their phone or computer and it automatically saves data and can be accessed by staff/providers. ### 2. Since integrating FormDr. into our practice, we have had very few issues with form completion. We also appreciate that we can create multiple forms to suit our needs and have the ability to share forms via email and text messaging. There customer support has been very helpful when we have needed them. **What do you dislike about FormDr?** We are a bit old school and would like to be able to call a help desk for support. No more asking for insurance cards and ID's at the front desk. We can easily obtain, save and transcribe patient information. **What do you dislike about FormDr?** There really is no downside to Formdr. We love it! **What problems is FormDr solving and how is that benefiting you?** We can instantly send intakes to patients that summarizes the information we need, their appt date and time and formdr will even send reminders to patients who have not completed their intake. We also have insight into patient activity like if the patient opened the link(s) and/or if it was completed. ### 5. Incredible support team **Rating:** 5.0/5.0 stars **Reviewed by:** Verified User in Insurance | Small-Business (50 or fewer emp.) **Reviewed Date:** May 23, 2024 **What do you like best about FormDr?** When you don't have FormDr you have to deal with a litany of issues that cause inconvenience such as not being able to read a client's handwriting, or risk the client leaving a question blank. Then you have to go back and forth waiting to get that information clarified or filled in. With FormDr, you can make fields required. You can send the forms to the client securely via email or text which is what most people would probably prefer anyway. Most importantly, the support team is top notch. I am so grateful for all of their help and patience in getting our packets set up. They have been absolute heroes. If you need something done, they will get it done reliably and with high quality. **What do you dislike about FormDr?** Form building is not the most user friendly. It seems uneccessarily complicated. While there are a lot of things that you can do, there are also several things that you can't do. For example, you can add a widget for clients to upload files, but you cannot add a widget to upload your own files (such as resources or brochures). The calculation widget is complicated and very picky. The help guide in the FormDr is terrible. All it does is say what something is, or how to find it, but not what it does exactly or how it works. **What problems is FormDr solving and how is that benefiting you?** Problems: Can't read client handwriting. Clients do not fill out application completely and leave questions blank. Non-FormDr intake forms are not uniform across all of our group clients. Getting paperwork back from employers can be a hassle and potentially require mass scanning or someone might be missed by accident. FormDr resolves all of these and more. It is taking forever to get it set up, but it will be worth it. ### 6. That's it. Nothing else good to say about it. **What do you dislike about FormDr?** We really leaned into using the product for a busy clinic. It is buggy and doesn't work. Was given the green light from development team that the bugs were fixed. Then, within an hour, it kicks all the users out and when we tried to log in it said all accounts were disabled. Called and mysteriously no one answers. **What problems is FormDr solving and how is that benefiting you?** It has created nothing but issues and hasn't solved anything. It has the potential to solve a lot of our issues like reducing phone calls etc, but at this point is has done nothing but frustrate patients and end users. ### 7. One can edit them and manage them per their need to some extent. **What do you dislike about FormDr?** The site issues are one to be worried about as most data gets lost and the screen gets stuck. Last I used FormDr for one our clients and the customer service was unreachable through any means. I havent used it for a long time but hopefully the CS would have improved. **What problems is FormDr solving and how is that benefiting you?** We used FormDr for one of our clients to send surveys to patients and get their review on the process of the overall experience.
