We started using FormDr in 2024 and I regret not using them sooner. We utilize their platform to send our new patient paperwork and other forms to patients. It has been a wonderfully easy platform to use and learn. Our patients no longer tell us they have troubles opening documents or that they are unable to complete our new patient paperwork. The ONLY thing we have found to be downside (but really not) is that you are only able to search by patient name for records. We are still able to pull up patient data by last name and/or first name! Review collected by and hosted on G2.com.

We like that we can easily share our forms with our patients. Since integrating FormDr. into our practice, we have had very few issues with form completion. We also appreciate that we can create multiple forms to suit our needs and have the ability to share forms via email and text messaging. When you don't have FormDr you have to deal with a litany of issues that cause inconvenience such as not being able to read a client's handwriting, or risk the client leaving a question blank. Then you have to go back and forth waiting to get that information clarified or filled in. With FormDr, you can make fields required. You can send the forms to the client securely via email or text which is what most people would probably prefer anyway. Most importantly, the support team is top notch. I am so grateful for all of their help and patience in getting our packets set up. They have been absolute heroes. If you need something done, they will get it done reliably and with high quality. Form building is not the most user friendly. It seems uneccessarily complicated.

While there are a lot of things that you can do, there are also several things that you can't do. For example, you can add a widget for clients to upload files, but you cannot add a widget to upload your own files (such as resources or brochures). The calculation widget is complicated and very picky. The help guide in the FormDr is terrible. All it does is say what something is, or how to find it, but not what it does exactly or how it works. We really leaned into using the product for a busy clinic. It is buggy and doesn't work. Was given the green light from development team that the bugs were fixed. Then, within an hour, it kicks all the users out and when we tried to log in it said all accounts were disabled. Called and mysteriously no one answers. Review collected by and hosted on G2.com.

FormDr is HIPAA-complain, which means that sensitive patient information is kept secure and confidential. FormDr eliminates the need for manual data entry and reduces the time spent on administrative tasks. Review collected by and hosted on G2.com.

Some users may find the platform's form templates and customisation options to be limited. FormDr relies on technology to function, which means that if there are issues with the platform or the user's device, they may not be able to access the forms they need.
