## FormAssembly Reviews
  ### 1. FormAssembly Review

**Rating:** 4.5/5.0 stars

**Reviewed by:** Verified User in Education Management | Small-Business (50 or fewer emp.)

**Reviewed Date:** March 30, 2024

**What do you like best about FormAssembly?**

What I like most about FormAssembly is it's customization aspects and how it is able to integrate seamlessly with other systems.

**What do you dislike about FormAssembly?**

The only thing I could complain about this system is when human error comes into play and I can't build out a validation rule.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is providing us with a way to administer forms while also collecting the information the forms provide and automatically transferring it to our CRM platform.

  ### 2. A powerful, must have tool especially if you're also using Salesforce.

**Rating:** 5.0/5.0 stars

**Reviewed by:** Ananda R. | Small-Business (50 or fewer emp.)

**Reviewed Date:** June 12, 2023

**What do you like best about FormAssembly?**

FormAssembly is an easy-to-use tool whose power extends far beyond its own platform because of its powerful connectors. It also has extensive learning and support materials online and excellent support customer service.

**What do you dislike about FormAssembly?**

If you have a form that has gotten thousands of responses or more, the results can be slow to load. Their Customer Support Team used to be available on chat but now it always seems to be listed as full and you have to reach out via email.

**What problems is FormAssembly solving and how is that benefiting you?**

Gives us an easy way to create completely customizable applications, surveys, registrations, payment processing, etc. And the most powerful part is it's connectors to Salesforce and our ability to send out prefilled URLs via email.

  ### 3. Great flexibility and excellent customer service

**Rating:** 5.0/5.0 stars

**Reviewed by:** Verified User in Non-Profit Organization Management | Small-Business (50 or fewer emp.)

**Reviewed Date:** June 13, 2023

**What do you like best about FormAssembly?**

It's a really flexible tool - the added complexity means an exponential set of options, in comparison with more intuitive programs. The help documents are incredibly helpful and include screenshots and gifs, and there are sample codes for common scenarios, etc. Where the help documents don't meet my needs, I've found the customer service team to be really responsive and helpful and have sent screenshots and recordings, as well as refer me through to other colleagues for more help. If you give them permission, they will go into your account and clone the form with issues, so that they test where it's going wrong. You can't overestimate the added value of having this level of support available.

**What do you dislike about FormAssembly?**

It took a little while to learn. We have used Smart Survey in the past, which is more drag and drop and intuitive for most people. It also requires basic coding if you want to put responses from different fields into the same Salesforce field, for example. Or if you want to create any conditional rules. So it isn't easy to pick up for your average office worker. Sometimes, when sending info through to Salesforce, error messages are shown to the end user, which isn't ideal, so be careful how you set up connectors.

**What problems is FormAssembly solving and how is that benefiting you?**

We wanted a flexible tool to build online forms for gathering data and sending it to Salesforce - lead generation, newsletter sign-ups, application forms, forms for other types of info, donation forms connected to Stripe. Form Assembly does all these things with built-in connectors to both Salesforce and Stripe (amongst others which we haven't really used). It has allowed us to make sure that all the metadata we wanted was fed into the CRM-system, for example, instead of having a Mailchimp newsletter sign-up form, we have a Mailchimp integration and we can ensure that we record how, when, where and the wording of the contact consent directly into Salesforce, which then triggers Mailchimp subscribe actions. Likewise, with donations, we now have a seamless integration with Stripe and Salesforce allowing us to send through metadata based on responses, which has reduced the manual import/export of lots of information. Our application forms used to be Microsoft Word documents which people emailed to us and were manually entered on Salesforce. This is now an automatic process saving countless hours of processing time.

  ### 4. My answer for almost everything

**Rating:** 5.0/5.0 stars

**Reviewed by:** Mark A. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 08, 2023

**What do you like best about FormAssembly?**

I found the documentation very user-friendly, making it quick and easy to get started using FormAssembly for even complex data capture tasks. It is my 'go-to' solution for almost all data capture needs, from lead generation to surveys and even communication preferences.

**What do you dislike about FormAssembly?**

I would love to be able to document-build, capturing field values entered earlier in the form to personalise, like using field tags in text and labels, i.e. "So, <<tfa_1>> tell us more about why you are interested in this..." where tfa_1 would be the first name. As far as I can tell, this can only be done on a multipage form.

**What problems is FormAssembly solving and how is that benefiting you?**

We help students in Scottish universities get paid internships with international companies. We use FormAssembly to capture applications to our internship program and the internship role description from our hosting companies, saving hundreds of work hours. We also have forms on our website where potential host companies can register their interest in joining our programme, giving us a significant pipeline to work with. As a small team in a non-profit organisation, we need all the help we can get to drive efficiency. This allows us to get on with helping these students get the best start in their careers to become Scotlands' next generation of business leaders, creating new jobs and a better Scotland.

  ### 5. I find it very good, but has a problem dealing with dates in non-American format, and not fixed.

**Rating:** 4.5/5.0 stars

**Reviewed by:** John C. | Mid-Market (51-1000 emp.)

**Reviewed Date:** February 07, 2023

**What do you like best about FormAssembly?**

Quite easy to use, has a great connector with Salesforce, quite versatile, but I haven't used it to its full potential, as there has not been the need as yet. Versatile with the custom code and functions.

**What do you dislike about FormAssembly?**

Firstly, its datevalue function does not know how to deal with dates in the dd/mm/yyyy format. I have raised this issue many times in a support case and been fobbed off basically. To me, I thought getting your date functions correct is basic, especially when you offer British format (and of course you need to). I have had to create so much custom code to change the British format to the American, just to make conditional statements dealing with dates work.

Another date issue is date validation. If a person enters a yy instead of yyyy in a date field, at some cutoff point, it will assume 20yy, and not 19yy. When I first raised this issue, I think the cutoff point was between 49 and 50 (or maybe 50 and 51)

You can refer to my support cases for further detail.

Not so much a dislike, but would be good to be able to use the copy option from one form to another, instead of only within the same form.

**What problems is FormAssembly solving and how is that benefiting you?**

Most markedly, the ability to connect to Salesforce saves so much time.

  ### 6. FormAssembly Product Owner Review

**Rating:** 4.0/5.0 stars

**Reviewed by:** Brandi H. | Mid-Market (51-1000 emp.)

**Reviewed Date:** February 03, 2023

**What do you like best about FormAssembly?**

Easy to use platform with the ability to create consistent themes and questions on the fly. The biggest feature asset for our organization is the Salesforce Integrator!

**What do you dislike about FormAssembly?**

Questions are not as customizable or click-of-a-button programmable as other survey platforms. For example, it would be helpful to have: programmable text fields/formulas within question text and the ability to program aliases into repeated questions to prevent the need to create multiple condition based questions (instead one question with repetition logic based on the aliases/responses from other questions).

Also, have the ability to be able to "re-review" or "re-approve" forms with Salesforce connectors if there are edits. Context: we have two teams supporting on FormAssembly and Salesforce without checks in place the connectors could break causing some of major forms to go down. Having an extra review option in place if anything in the form changes would be a lifesaver!

**What problems is FormAssembly solving and how is that benefiting you?**

Our program and development teams use FormAssembly for student applications, lead development, survey development/online form building and support organizationally no/low-code platforms, which this tool easily provides.

  ### 7. Love it! Especially if you're integrating with Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Emilee S. | Mid-Market (51-1000 emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

- Form design flexibility
- Salesforce integration is THE BEST around
- Form pre-fill capabilities
- Heavy backend customization where we need it most
- Not restricted with as many limits as other form tools
- Workflow! Seriously, amazing feature. Sometimes I don't even need it because your out of the box connectors are complex and fit the need
- Customer support is timely and great to work with
- Admin support docs - perfect. I've never worked with a tool that had resources as organized as FormAssembly

**What do you dislike about FormAssembly?**

- Lack of Outlook 365 integration (you have Google, but sending results to a 365 sheet would be AMAZING)
- The weirdness of requiring sharing forms/templates to other admins in the tool
- I can't save as a draft. Or at least I haven't figured that out without cloning a form. Would be nice when testing a major change to a form if I could test it before it would impact the live form

**What problems is FormAssembly solving and how is that benefiting you?**

1. A form builder that connects and works directly with SFDC but is available to non-SFDC users and can be used externally wherever we need it
2. Customized dynamic form capabilities for our clients utilizing SFDC data to determine what should be asked creating a seamless experience for all

  ### 8. Excellent productivity and data collection tool for our small non-profit

**Rating:** 4.5/5.0 stars

**Reviewed by:** Eileen M. | Program Director, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 07, 2023

**What do you like best about FormAssembly?**

We love the ability to collect data from our students and have it mapped directly into our Salesforce instance. Creating forms is easy and connecting them to SF is possible with the tutorials and help available through the form assembly platform.

**What do you dislike about FormAssembly?**

The only challenging part of Form Assembly for us is the setup. We are a small business and don't have a tech department to handle stuff. We have been pleased with the tutorials available to do minor stuff but for major forms/mapping, we've needed to hire an outside consultant.

**What problems is FormAssembly solving and how is that benefiting you?**

We used to have to send forms, collect the data and then input it into individual Salesforce accounts or spreadsheet, its awsome that form assembly can be built to automatically input the data where it needs to be in our Salesforce instance.

  ### 9. FormAssembly is great.

**Rating:** 4.5/5.0 stars

**Reviewed by:** Dan W. | Manager of Information Technology and Data Systems, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 15, 2022

**What do you like best about FormAssembly?**

My favorite part about FA is the ease of creating forms. There are loads of resources in their online help platform, and mapping to Salesforce is a massive win for our organization.

**What do you dislike about FormAssembly?**

I dislike that when something does go wrong, the log that shows errors can be vague. This is a declarative tool and, therefore, should have easy-to-understand errors. If the error points to something that is unclear, some help text showing possible reasons for erros this like would be beneficial.

**What problems is FormAssembly solving and how is that benefiting you?**

FA helps us solve our data quality issues. When we want to know information and keep it up to date with our clients, we use FA forms. I would say that there isn't a comparable tool out there, given all that FA can accomplish.

  ### 10. FormAssembly is the BEST!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Trish P. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

FormAssembly makes it easy to move data into Salesforce, no matter how complex the data. Their team is also super responsive when you have questions, issues or just need to learn more.

**What do you dislike about FormAssembly?**

The only downside I can think of is that it's a bit pricey. Because I'm a FormAssembly partner, I'm able to direct the nonprofits I work with to all the discounts that are available, though, so we end up with exactly the tool we need at a price my nonprofits can afford.

**What problems is FormAssembly solving and how is that benefiting you?**

Getting data from their constituents, creating registration forms for events and conferences, and learning more about who their audience is-- my nonprofit clients just can't find another tool that does it all, with ease.

  ### 11. Reliable and would recommend

**Rating:** 5.0/5.0 stars

**Reviewed by:** Tabitha P. | Vice President Operations, Small-Business (50 or fewer emp.)

**Reviewed Date:** September 11, 2023

**What do you like best about FormAssembly?**

Reliability and connects well with our CRM and website-building tool.

**What do you dislike about FormAssembly?**

It is pricy, but after seeing how reliable, the price is right.

**What problems is FormAssembly solving and how is that benefiting you?**

we were previously using WP forms which did not connect to our Salesforce wich cause loss of revenue and time

  ### 12. A good tool, a bit expensive with non existant support

**Rating:** 1.5/5.0 stars

**Reviewed by:** Arnaud C. | Head of data and platforms, Mid-Market (51-1000 emp.)

**Reviewed Date:** November 21, 2022

**What do you like best about FormAssembly?**

Does what it says on the tin. supercharged form builder that simplify integrations with a lot of tools.

**What do you dislike about FormAssembly?**

Support is non-existant. An account manager follows you until you sign up, but past that point, you are on your own. If you have an issue with billing, you are on your own. The support redirects you to a different department with a dedicated email address and you can send as many emails as you want, you won't receive a response. Escalating does not help; you will not receive a response either.
On top of that, the product is rather pricey. the market is full of similar tools and it seems that it is mostly popular amongst Salesforce users who are used to paying a premium for everything. That would be fine if the support was reflecting that...

**What problems is FormAssembly solving and how is that benefiting you?**

No code form builder

  ### 13. Non-Profit Integration Into Salesforce

**Rating:** 3.5/5.0 stars

**Reviewed by:** Keith A. H. | Director, Impact and Operations, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

We have been able to collect different types of information (surveys, invitations, interest) and automate their input into Salesforce for tracking and analysis. It has helped us streamline many of our processes and ensured we're maintaining a single source of truth in one database rather than taking time to upload data on a regular basis.

**What do you dislike about FormAssembly?**

There's a steep learning curve in creating connections. Making a small mistake at the onset can cost hours of redoing work.

There doesn't to be a way to be notified of errors in the connection - so you have to manually look through logs to resolve. You can't clear the log so you end up going through to see if you previously cleared the error.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly allows us to collect various types of information and integrate the information directly into Salesforce. This allows more time for analysis and less time manually shifting data.

  ### 14. A great tool to quickly build Questionnaire

**Rating:** 4.5/5.0 stars

**Reviewed by:** Roopa B. | Mid-Market (51-1000 emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

- The point-and-click tools used to build forms are very user-friendly.
- In the latest release FA team has added support bubbles to understand the functionality for using a particular option which is very helpful for beginners.
- I had a good experience with chat support agents who are knowledgable to understand the issue and provide assistance.

**What do you dislike about FormAssembly?**

It will be more valuable if more training materials (Ex: youtube videos) are posted on how to use various features and also for some complex functionality

**What problems is FormAssembly solving and how is that benefiting you?**

The filtering options in connectors are helping a great deal.

  ### 15. Nimble form development tools to meet your business needs.

**Rating:** 5.0/5.0 stars

**Reviewed by:** Jeffrey J. | Senior Technology Executive, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

The simple interface, robust connection options, and flexible configuration sets FormAssembly apart from other tools I've used. The Salesforce connector alone has allowed me to deliver solutions to meet a variety of needs.

**What do you dislike about FormAssembly?**

The inability to test forms in a test environment and push to production has provided some limitations on already published forms.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly currently meets many needs. It serves as a tool to capture reports from the community, the application for admission to a university, a post-graduation survey tool, and much more.

  ### 16. FormAssembly integrated with Salesforce is an essential tool for us

**Rating:** 4.5/5.0 stars

**Reviewed by:** Sue L. | Salesforce Admin, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

The ability to get data into Salesforce while eliminating duplicates and setting up multiple standard and custom objects.
I would say it is moderate to easy to use.
Customer service is excellent

**What do you dislike about FormAssembly?**

It is slow to add every field and connection in the Salesforce connector.
Also, the only way to have forms in multiple languages is to have multiple forms

**What problems is FormAssembly solving and how is that benefiting you?**

Student Enrollment, Seasonal Applications, Surveys, Assesments, duplicate elimination

  ### 17. Powerful Integrated Form Creation Tool

**Rating:** 4.5/5.0 stars

**Reviewed by:** Alex S. | Director, Data Management, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 16, 2022

**What do you like best about FormAssembly?**

FormAssembly does an excellent job with Salesforce in particular. The pre-fill and post-submission connectors are exceptional and allow us to supplement or expand website functionality in very valuable ways. That it's so configurable to these ends is without a doubt the most valuable element of this tool for us.

**What do you dislike about FormAssembly?**

Connectors can be confusing to configure and troubleshoot. Other issues are conditional and dynamic fields - specifically that you cannot make things conditional based on text values easily. This makes it challenging to architect dynamic forms that don't use structured data types.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is the tool we use to bridge gaps in website functionality. It can serve as a testing ground for features that we are considering making a part of our suite of website features - things like registration to events as a group, for example. The automation components allow it to be used as an MVP product but still integrate.

  ### 18. A Pros and Cons list of FormAssembly

**Rating:** 3.5/5.0 stars

**Reviewed by:** Sarah S. | Web and Digital Communications Manager, Mid-Market (51-1000 emp.)

**Reviewed Date:** May 19, 2022

**What do you like best about FormAssembly?**

* Pretty good functionality and features for the price
* Decent documentation in the Support KB
* Ability to import common data sets
* Ability to embed forms on websites is great
* There's a good variety of connectors to external systems, especially Salesforce
* You can do a lot of customization with the form appearance
* You can implement a wide range of formulas and calculations for even more powerful functionality and customization
* There's a pretty good form builder interface that is accessible to most users

**What do you dislike about FormAssembly?**

* While there's a wide variety of connectors available, they are not always the most robust. There are some wildly GLARING gaps in functionality that are incredibly frustrating, especially when it comes to the Salesforce connector and some of the finance connectors
* In my opinion, you do need someone with technical expertise if you want to connect these forms to an external system in order to understand how the connectors work. You also definitely should have someone with knowledge of HTML/CSS to really be able to customize the appearance of the form because unless you go into the code to tweak things to exactly how you want, the form builder wizard does not always give you the best output.
* Form Assembly's customer support isn't the most helpful and the Knowledge KB could always be expanded/improved

**Recommendations to others considering FormAssembly:**

Do your due diligence and map out ALL of your business use cases and workflow processes first

**What problems is FormAssembly solving and how is that benefiting you?**

We use FA forms for event registration, newsletter subscriptions, and even course applications--sometimes all within the same form. It's been extremely useful to be able to connect forms directly to our Salesforce org to immediately capture customer data (rather than having to constantly manually import these contacts into Salesforce)

  ### 19. Supercharge Salesforce effortlessly collecting and integrating form submissions

**Rating:** 4.5/5.0 stars

**Reviewed by:** Joseph U. | Owner, Small-Business (50 or fewer emp.)

**Reviewed Date:** August 11, 2022

**What do you like best about FormAssembly?**

The upsides of using Form Assembly is easily the degree to which you can customize how form data gets populated in Salesforce. There are few non-programming options available within Salesforce to collect external information. And even those options such as Web to Lead doesn't allow you anywhere close to the customizability you get from Form Assembly. That in and of itself is worth the price of admission for many organizations. But Form Assembly doesn't stop there. Not only can you collect data and store in Salesforce in limitless ways but you can also incorporate other services as well within the same process. For example, collecting an online donation via a webform, having Stripe collect the funds and seeing that donation entered into Salesforce all without a line of code being required.

**What do you dislike about FormAssembly?**

I really haven't had any huge problems as the experience has overwhelmingly been positive for me. The issue that I encountered a few years back was creating forms that collect donations and being flagged by the algorithm as requiring review. Its been a couple of years since that has happened so perhaps it has been fixed or improved.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly makes collecting information via the web and integrating it with Salesforce and other 3rd party services a cinch. Without a service like this, you would need to program the solutions which take a far greater amount of time and/or expense for an organization that doesn't have development resources. Even being a developer I recommend using FormAssembly as it saves so much time and works reliably.

  ### 20. Best form tool I've used - great Salesforce integration

**Rating:** 5.0/5.0 stars

**Reviewed by:** Marya C. | Salesforce Administrator, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 10, 2022

**What do you like best about FormAssembly?**

The Salesforce prefill and Submission connectors allow for a sophisticated Salesforce administrator to do magical things with forms. I've set up single forms that serve multiple business processes and save our staff SO MUCH data entry, back and forth communications, and prevent data entry errors.

**What do you dislike about FormAssembly?**

There are still a few features that you need to do a little creative problem-solving to make the most of. For example, in order to send a PDF to the person who completes the form, I had to do a workaround that is meant to send a PDF to our internal users. FormAssembly told me how to do this, so I'm not sure why they haven't built this feature into the options for the auto-responder.

**What problems is FormAssembly solving and how is that benefiting you?**

The staff in my organization asked for a "Portal" that would enable our volunteers to have a window into their current status and the steps they need to complete to be fully cleared to volunteer. They thought it was a "pie in the sky" request, but FormAssembly's prefill connector essentially made this possible.

  ### 21. User friendly and robust!

**Rating:** 4.5/5.0 stars

**Reviewed by:** Verified User in Non-Profit Organization Management | Mid-Market (51-1000 emp.)

**Reviewed Date:** June 06, 2023

**What do you like best about FormAssembly?**

It is very user-friendly but can be made more complicated for more complicated needs.

**What do you dislike about FormAssembly?**

If you try to use section columns, they run off the page when viewing the form on a phone. The same issue occurs when viewing a Likert matrix. It would be nice if this could be fixed.

**What problems is FormAssembly solving and how is that benefiting you?**

My clients use this for everything from applications to quick surveys to event sign-ups. It is nice that one tool can be used for so much!

  ### 22. We are bale to successfully integrate with Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Mayra G. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

We love to connect it to Salesforce, and it is dynamic when customizing the form for our customers. It is a little bit of a learning curve, but it is easy to be proficient here.

**What do you dislike about FormAssembly?**

I want to be able to integrate Java Script in a better way. I would love a better editor that is more dynamic since it is hard to create more complex forms because of this feature.

**What problems is FormAssembly solving and how is that benefiting you?**

We can create enrollments for our programs automatically, we get our Database super clean, and it is easy to customize what kind of data we require.

  ### 23. Best tool to integrate into Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Pedro P. | Gerente de Customer Journey, Mid-Market (51-1000 emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

The salesforce connector is excellent. It allows you to build complex flows and update records easily. Also, the possibility to calculate and formulate fields is very useful.

**What do you dislike about FormAssembly?**

That it does not have the ability to integrate with other tools. I would really like a connector with Zapier.

**What problems is FormAssembly solving and how is that benefiting you?**

Automatically create and update records in salesforce.

  ### 24. CRM Connection is a Lifesaver

**Rating:** 3.5/5.0 stars

**Reviewed by:** Jamed D. | Digital Marketing Manager, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 22, 2022

**What do you like best about FormAssembly?**

The connection and the depth that you can send values to our Salesforce.
All of the question/response logic that you need.
The simplicity of being able to create new forms and being able to duplicate existing forms.

**What do you dislike about FormAssembly?**

I have limited understanding of Syntax so it would be good if there was another way to manage this
When forms are embedded onto our websites the styling is muddled - would be great if it was easier to manage styling from TFA end
I would love it there was the ability to create custom quizzes (or make it easier to do) that presented question & answer, and then right/wrong before going to next question.

**What problems is FormAssembly solving and how is that benefiting you?**

Creating and optimising online forms for CRM acquisition and lead generation
Online engagement to drive longer time on website

  ### 25. Form Assembly - no clicks - no code - pretty powerful!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Candice C. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

Flexibility in updating information from Form Assembly to Salesforce. Also used form handlers for Pardot which was super helpful.

**What do you dislike about FormAssembly?**

Some conditions in the pre-fill don't work. The repeatable sections need to do more than just repeat

**What problems is FormAssembly solving and how is that benefiting you?**

Create forms easily and don't have to configure them to be mobile friendly.

**What do you dislike about FormAssembly?**

Some limitation exist in workflow, including the inability to access easily created or updated records from previous steps.

  ### 27. Easy to use app to help my org collect data

**Rating:** 5.0/5.0 stars

**Reviewed by:** Pilar D. | Director of Operations, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

The ease of creating professional looking surveys and the ease of adding to emails and website. Lots of Intergration with other applications.

**What do you dislike about FormAssembly?**

Connecting to other applications like Salesforce could be a challenge, but not a dealbreaker.

**What problems is FormAssembly solving and how is that benefiting you?**

Ability to create surveys to collect data.

  ### 28. Great way to manage data going into Salesforce

**Rating:** 4.5/5.0 stars

**Reviewed by:** Rebecca D. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

The conditional question capabilities are amazing and far surpass surveymonkey and googleforms.
The Salesforce connectors are invaluable!

**What do you dislike about FormAssembly?**

Some of the backend UI are dated- like the connectors.

**What problems is FormAssembly solving and how is that benefiting you?**

I use formassembly primarily to flow information into Salesforce live for registration, signups and surveys

  ### 29. Great tool for Salesforce Admins!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Lani B. | Senior Vice President, Small-Business (50 or fewer emp.)

**Reviewed Date:** August 09, 2022

**What do you like best about FormAssembly?**

I love how easy they make it to learn the platform. They even have their own Academy where you can do modules to earn badges, and get certified in the Platform. The Salesforce and Stripe connectors are awesome!

**What do you dislike about FormAssembly?**

Just that sometimes it takes a while to get an answer from support if I get stuck. It doesn't take that long, but when I get pressure from my boss to finish something it can be mildly frustrating to have to wait for support. Once I get help though it's amazing, and the reference articles are very thorough.

**What problems is FormAssembly solving and how is that benefiting you?**

Form Assembly provides an easy way for us to have custom forms on our site and take payments and get all of the information seamlessly into Saleforce so we can reference it later.

  ### 30. It works well. but their are more aesthetic platforms out there.

**Rating:** 4.0/5.0 stars

**Reviewed by:** Easton D. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 09, 2023

**What do you like best about FormAssembly?**

The conditional questions and Salesforce integration.

**What do you dislike about FormAssembly?**

The aesthetic. It could provide more options with themes.

**What problems is FormAssembly solving and how is that benefiting you?**

It provides a clean registration system for various events and forms. It benefits us by being an all in one registration solution.

  ### 31. Good Integrated tool with Salesforce for Forms

**Rating:** 5.0/5.0 stars

**Reviewed by:** Hiral S. | Senior Information Systems Analyst, Mid-Market (51-1000 emp.)

**Reviewed Date:** February 06, 2023

- An excellent tool for creating and integrating Dynamic Forms with Salesforce
- We can customize the back end as per our requirements
- We can use CSS with Forms to design the front end
- It has inbuilt Pre-Fill and Post-Submission Connectors

**What do you dislike about FormAssembly?**

Having conditional fields in the forms is a little tricky and confusing. They should also provide more customer support.

**What problems is FormAssembly solving and how is that benefiting you?**

It is easy to build forms where you need user input.

  ### 32. Great for collecting applications for out organization!

**Rating:** 3.0/5.0 stars

**What do you like best about FormAssembly?**

I like that we can continually count on FormAssembly to meet our need.

**What do you dislike about FormAssembly?**

My main dislike is that Forms don't look very appealing. They could look better.

**What problems is FormAssembly solving and how is that benefiting you?**

Applications for non-profit after school program.

  ### 33. FormAssembly was born in Salesforce

Technical capabilities, specially about the Salesforce connector.

**What do you dislike about FormAssembly?**

The actual form builder interface could be more user friendly and have more design options.

**What problems is FormAssembly solving and how is that benefiting you?**

Unifying processes and filling the gap between automation systems.

  ### 34. Solid Product and Customer Service

Customer service

**What do you dislike about FormAssembly?**

Not a direct connector to Salesforce for the e-signature component

**What problems is FormAssembly solving and how is that benefiting you?**

Huge save in data entry

  ### 35. FormAssembly Rocks

**Rating:** 5.0/5.0 stars

**Reviewed by:** Dustin B. | CRM & Data Administrator, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 22, 2021

**What do you like best about FormAssembly?**

I love its connection with Salesforce. I think it has a lot of great capabilities by utilizing integrations and formulaic generation. I also feel like the ability to customize the form is extremely useful. From CSS customization to just simple layout, it works for everything we need at my company. The Google Sheet integration is also a lifesaver so I can pass along some of the results of our forms to my coworkers without having to have them log in to FormAssembly. Great stuff to keep the team on the same page!

**What do you dislike about FormAssembly?**

I honestly don't have any big "dislikes". It seems to be a great platform for what I and my company use it for. The only issue I have had at all is that during the entirety of COVID, the Support option has been offline. It would be nice to contact someone at some point for quick help tips. I'm not really sure this is something that can/could be fixed, but I started working with FormAssembly around February-ish prior to the COVID, and then really got into it after COVID had gotten into full swing. So I am not really sure what the support situation was like prior to that.

**Recommendations to others considering FormAssembly:**

If you utilize Google Drive and/or Salesforce to collect information on your customers/contacts, I highly suggest using FormAssembly. Not only can you easily create forms to collect information (and payment) from customers, you can also easily integrate it into a website. It's great and extremely useful!

**What problems is FormAssembly solving and how is that benefiting you?**

The ability to register a bunch of students without having paper documents. It has really helped with the virtual world we have found ourselves in. The integration with Stripe and Salesforce also allows us to take payments and pass that information along to our Salesforce CRM.

  ### 36. Functional tool with easy Salesforce integration

**Rating:** 4.0/5.0 stars

**Reviewed by:** Ryan J. | Salesforce Administrator, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 23, 2021

**What do you like best about FormAssembly?**

The Salesforce Connectors which allow us to very quickly set up any sync with our Production or Sandbox environments.

**What do you dislike about FormAssembly?**

Not having an address lookup feature on the form. There is one through a paid third party product but it would be very helpful to allow users to input their address and have suggestions appear like those using Google maps integration. This is functionality that is extremely helpful and improves the customer experience. We have had this requested from multiple internal stakeholders and because of this there is a slight push internally to consider other tools or at least use in tandem with FormAssembly.

**Recommendations to others considering FormAssembly:**

Use the trial and see how you go. I recommend using the tool directly through their site rather than from within Salesforce.

**What problems is FormAssembly solving and how is that benefiting you?**

In the past we had an issue with only coded forms as this reduced the ability for other staff members to create and contribute to the building process. We can now do this thanks to the easy drag and drop editor. The Salesforce integration was a major issue with other tools that were considered and upon our trial we realised how simple this process was with FormAssembly.

  ### 37. Form Assembly made easy

**Rating:** 5.0/5.0 stars

**Reviewed by:** Rupinder K. | Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

It's a fantastic experience working with Form Assembly forms. I can create simple and user-friendly forms for my clients. Conditional field options help me create one form instead of two forms for the client.

**What do you dislike about FormAssembly?**

I like all about Form Assembly. Great experience with FA

**What problems is FormAssembly solving and how is that benefiting you?**

Intake forms for client

  ### 38. Easy to Design and Easy to Integrate

**Rating:** 5.0/5.0 stars

**Reviewed by:** Erik F. | Systems Director – Archdiocese for the Military Services, USA, Mid-Market (51-1000 emp.)

**Reviewed Date:** April 16, 2020

**What do you like best about FormAssembly?**

FormAssembly allows you to be creative in your form design and to integrate with Salesforce and other Apps.

**What do you dislike about FormAssembly?**

I wish FormAssembly would automatically create PDF version from the form design. Some of the connectors with SF are not as intuitive as you wish they were.
I would also recommend that the Instructions on how to integrate were numbered so that one can follow step by step.

**Recommendations to others considering FormAssembly:**

If you intend to use FormAssembly to integrate with SF, learn how to configure the connectors in first place and attention to the lookup configs as well.

**What problems is FormAssembly solving and how is that benefiting you?**

I use FormAssembly to integrate with Salesforce. We use it to open SF Cases from our constituents. FormAssembly has helped in the automation and improvement of a work process, reducing paper trail, phone calls, allow for on-time updates and transparency.

  ### 39. Very, very powerful solution for direct-to-Salesforce connection

**Rating:** 4.0/5.0 stars

**Reviewed by:** Trish V. | Marketing Manager, Small-Business (50 or fewer emp.)

**Reviewed Date:** August 10, 2022

**What do you like best about FormAssembly?**

I like that there are quite a few different integrations and the drag-and-drop format for form building is nice and simple. It is a very powerful solution for getting data directly into salesforce whether that's contacts, leads, or campaigns, I've had pretty good success and reliability in the solution.

**What do you dislike about FormAssembly?**

I would say the support line can be difficult. It is chat-only. I am a hands-on learner and wish I could call someone when I get stuck. I would also say that if you are using for Salesforce integrations, you need to have a really good understanding of logic rules and Salesforce principles about leads/contacts.

**What problems is FormAssembly solving and how is that benefiting you?**

It saves our sales team plenty of time when the information is directly imported into salesforce for demo requests.

  ### 40. Handles our custom application needs while offering great formatting options!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Verified User in Non-Profit Organization Management | Small-Business (50 or fewer emp.)

**Reviewed Date:** August 09, 2022

**What do you like best about FormAssembly?**

I most like the appealing formatting capabilities and brand customization. I also like how well it works with salesforce to send over information. I also like how easy some parts are to use.

**What do you dislike about FormAssembly?**

I dislike the lack of automation options available and the limited amount of integrations with various platforms like airtable. I also dislike that I can't end a form response immediately if someone chooses a particular survey answer choice. I also wish I could do more with font changes

**What problems is FormAssembly solving and how is that benefiting you?**

It's solving the need for a dynamic form to gather information and give important information. I am able to gather all of the info I need and make sure my volunteers have all that they need to know before submitting the form.

  ### 41. It does a decent job of building forms

**Rating:** 2.5/5.0 stars

**Reviewed by:** Sravanth G. | Enterprise (> 1000 emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

Easy integration with Salesforce, allows for conditional questions

**What do you dislike about FormAssembly?**

There are few key areas that could use improvement in terms of UX.

**What problems is FormAssembly solving and how is that benefiting you?**

Helps us create no-frills subscription forms and integrates with our CDP - Saleforce CRM.

  ### 42. Number One Tool for Constituent Surveys, Data Collection, and Salesforce Integration

**Rating:** 4.5/5.0 stars

**Reviewed by:** Daniel B. | Program Coordinator, Small-Business (50 or fewer emp.)

**Reviewed Date:** May 19, 2022

**What do you like best about FormAssembly?**

The Salesforce connector is flexible, robust and awesome. The number of use cases we use it for are too many too list. Customer support is great - although technical issues are rare, the team has always followed up promptly when we've encountered a problem.

**What do you dislike about FormAssembly?**

I wish FormAssembly had a "rank ordered picklist" question type. It would be nice to be able to sort forms in custom folders. A more robust text editor within forms would be nice as well.

**Recommendations to others considering FormAssembly:**

There's no other survey tool out there that integrates with Salesforce better.

**What problems is FormAssembly solving and how is that benefiting you?**

Intake forms for our organization - collecting information about new contacts and leads, and creating appropriate contact, account, and opportunity records in Salesforce. Huge constituent surveys - 100+ item questionnaires with flexible conditional options.

  ### 43. Excellent flexible & multifunctional tool!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Andrew M. | Analyst, Admissions Marketing, Mid-Market (51-1000 emp.)

**Reviewed Date:** May 19, 2022

**What do you like best about FormAssembly?**

Form Assembly has a deceptively simple UI full of design features to create the form you need. The connectors, esp pre-fill, are a lot of fun to use.

**What do you dislike about FormAssembly?**

Editing and saving a form change it in real-time. It needs the ability to change a form but not publish it right away.

**Recommendations to others considering FormAssembly:**

I highly recommend Form Assembly to anyone looking for an online forms tool to collect data for a wide range of use cases.

**What problems is FormAssembly solving and how is that benefiting you?**

We use it to create all our online forms related to the university admissions process. This includes applications, residence applications, references, supplements, and more. We currently have 56 active forms. It helps us collect data and connect it directly to where it needs to go in salesforce, across multiple objects.

  ### 44. Easy to Use and great functionality

**Rating:** 5.0/5.0 stars

**Reviewed by:** Mardo C. | Database Administrator, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 12, 2022

**What do you like best about FormAssembly?**

I like the integration with Salesforce which allow us to update fields in every object as well as create new records based on different conditions.

**What do you dislike about FormAssembly?**

I don't like that when you add new content to the form, the new content always goes to the bottom of the page.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly allows us to expand our salesforce platform for those who have a limited license. Throughout the connectors, users can create opportunities and products in a simple and effective way.

  ### 45. Easy application that allows for quick professional survey!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Pilar D. | Senior Operations Manager, Small-Business (50 or fewer emp.)

**Reviewed Date:** May 19, 2022

**What do you like best about FormAssembly?**

An easy application we added to provide professional surveys. Easy to create, using a template, copying an existing survey and the different ways of displaying my questions and answers. Working in a small office, you don't need to be an IT expert to start on Form Assembly.

**What do you dislike about FormAssembly?**

I would love a way to be able to save a survey as a PDF or word doc once a survey has been created. Would love to see about adding users to an account.

**Recommendations to others considering FormAssembly:**

It is a cost-effective way to create professional-looking forms to share with clients, board members and staff.

**What problems is FormAssembly solving and how is that benefiting you?**

We are collecting data from our clients quicker and easier. We can review responses, notify stakeholders and integrate with our CRM.

  ### 46. Easy-to-use platform for forms in Higher Education

**Rating:** 4.5/5.0 stars

**Reviewed by:** Verified User in Higher Education | Mid-Market (51-1000 emp.)

**Reviewed Date:** August 09, 2022

**What do you like best about FormAssembly?**

FormAssembly is extremely easy to use and very simply connects to both Salesforce and Google Sheets. The best part about FormAssembly is their documentation and customer service; both are incredibly helpful and provide detailed support.

**What do you dislike about FormAssembly?**

One downside of FormAssembly is that the new Workflow tool is a bit slow to load when building a Workflow. This is a brand new feature, so I am confident this will improve over time.

**What problems is FormAssembly solving and how is that benefiting you?**

For a higher education institution, FormAssembly allows us to get student information directly into our Salesforce platform, without the need to upload files from a separate source. Previous forms with responses documented in various departments are now housed in one place and related to the student.

  ### 47. I will always use and recommend FormAssembly!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Ananda R. | Managing Director of Digital Products, Mid-Market (51-1000 emp.)

**Reviewed Date:** May 24, 2022

**What do you like best about FormAssembly?**

FormAssembly is incredibly easy to use and comes with extremely robust features like powerful connectors to Salesforce and payment processors and dynamic prefill URLs.

**What do you dislike about FormAssembly?**

The UX/UI could be improved for less "techie" users.
Improve the ability to create questions for respondents to easily rate answer options (like Matrix or Likert style answers).

**Recommendations to others considering FormAssembly:**

Get FormAssembly! FormAssembly is an incredibly powerful tool that can be used in many different ways to support various needs and operations of your organization.

**What problems is FormAssembly solving and how is that benefiting you?**

We replaced an entire student application system with FormAssembly!
We also use it to facilitate or email preference center which serves more than 600,000 subscribers.
We use it for all of our internal and external survey needs.

  ### 48. Great form builder with a lot of features

**Rating:** 4.0/5.0 stars

**Reviewed by:** Steve S. | Customer Experience and Marketing Manager, Small-Business (50 or fewer emp.)

**Reviewed Date:** May 24, 2022

**What do you like best about FormAssembly?**

The integration with Salesforce was very in-depth with a lot of features. We could process our form submissions in a lot of ways to create leads or contacts, check for duplicates and much more. The interface was easy to use. Having such a robust Salesforce integration was a big reason we used FormAssembly. Also, their options for creating the form were very robust as well. We're now using Pardot, and the functionality is extremely lacking compared to the features of FormAssembly, such as creating groups of fields, hide/show based on other selections, multi-step forms, etc. Also, because the forms are each on their own page, and we can embed Google Tag Manager, it allows us to insert tags on our form pages, which was very helpful as well.

**What do you dislike about FormAssembly?**

The styling of the forms was a bit tedious. I like how they give you the option to write your own CSS, but knowing which elements to change and having so many default styles to override was a bit annoying. I had to go back and forth between viewing the code and writing the styles many times to get it right.

**What problems is FormAssembly solving and how is that benefiting you?**

While we were using FormAssembly, our websites were built in Wix, which has a terrible form system and very few integrations. FormAssembly allowed us to embed complex forms that are easily editable by anyone on our team, and then integrate directly with Salesforce so our reporting is always up-to-date and accurate. This allowed us to capture all the necessary information for quotes based on who is completing the form.

  ### 49. Pretty great product!

**Rating:** 4.0/5.0 stars

**Reviewed by:** Elaine J. | Data Associate, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 25, 2021

**What do you like best about FormAssembly?**

The Salesforce integration is so helpful - no other system that we have tried works so well. I also like the webinars that Form Assembly has been putting on recently.

**What do you dislike about FormAssembly?**

I wish there were more options for customization within form building in Form Assembly. Specifically, I'd like options to be able to click to customize my form (rather than needing to use code). Although the forms are functional, our users don't love how they look. One thing we would love is a drop-down menu of font options - we currently aren't using our brand font and that's not ideal.

**Recommendations to others considering FormAssembly:**

If you are using Salesforce and have reason to collect information from individuals (that you would like stored in Salesforce), Form Assembly makes that SO easy to achieve. I would definitely recommend looking into it, at least. Form Assembly has several other integration options (such as Google Docs) which can also be helpful! For example, some of my colleagues don't love Salesforce and are quite attached to Google Docs. Form Assembly allows me to simultaneously get the info I need into Salesforce AND into a Google Doc for easier reading for my colleagues.

**What problems is FormAssembly solving and how is that benefiting you?**

With Form Assembly, we are able to reduce the number of duplicate records that get created in Salesforce - this is a huge need. It also allows us to gather information from external folks and have it immediately stored in Salesforce (no additional uploads required). This allows for cleaner data and fewer folks having Salesforce licenses - which is ideal in this scenario.

**Official Response:**

> Hi Elaine! Thank you so much for your review. I wanted to reach out to let you know that through theme customization and Google Fonts, you can add a wide range of font styles to your form! This method does require code, but we just updated a tutorial on how to do this in just a few steps. Please let us know if you have further questions!

  ### 50. Great form solution!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Emily P. | Salesforce Admin, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 09, 2022

**What do you like best about FormAssembly?**

FormAssembly has enabled our institution to create and connect forms to multiple Salesforce orgs as well as Google forms.

**What do you dislike about FormAssembly?**

While the company is working to create customer-recommended solutions, one function that would be helpful for our team is a better way to organize our forms.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly benefits our team by enabling us to focus on target audiences utilizing single-sign on functions, community sign-in functions, and guest user functions.
