**What do you like best about FormAssembly?**

I like that FormAssembly is its flexibke and seamlessly integrates into Salesforce. We use it for everything from event registration to complex admissions workflows, and the conditional logic and workflow features allow us to automate processes that would otherwise require a lot of manual effort. The form builder is easy to use, making it simple to build and update forms as our business needs change.

**What do you dislike about FormAssembly?**

Managing large workflows can become cumbersome, especially when there are many conditional rules. I would also like to see more detailed error messages to make troubleshooting easier. While the platform is powerful, there are a few administrative tasks that could be more intuitive and streamlined.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly helps us automate admissions and recruitment processes while ensuring data flows directly into Salesforce. It has reduced manual data entry, improved data accuracy, and allowed us to quickly build forms and workflows to support changing business needs. **What do you like best about FormAssembly?**

What I like best about FormAssembly is its versatility. I regularly find myself turning to it to solve a wide range of business needs, from building forms and surveys to creating more complex workflows that help streamline internal processes and improve how information is collected and shared.

The Salesforce connector has been especially valuable because it allows us to send data directly into Salesforce, reducing duplicate entry and helping ensure our records stay accurate and up to date. The Google Sheets connector has also been a game changer when we need to share real-time submission data with other teams without creating additional manual reporting work.

Overall, FormAssembly has helped us automate processes that would otherwise require multiple tools, spreadsheets, and manual handoffs. It gives us the flexibility to build solutions quickly while still supporting more advanced integrations and conditional workflows as our needs evolve.

**What do you dislike about FormAssembly?**

What I dislike most about FormAssembly is the difficulty of getting timely, hands-on support when an issue requires help from a real person. The help articles and self-service resources are useful, but there are times when troubleshooting a complex form, connector, or workflow requires direct assistance.

Because phone support is not available, live agents are rarely accessible, and email responses can sometimes take several days, resolving urgent issues can be slower than it should be. A more reliable live chat option or faster response times for support requests would significantly improve the overall customer experience.

**What problems is FormAssembly solving and how is that benefiting you?**

Before using FormAssembly, many of our processes relied on emails, spreadsheets, duplicate data entry, and manual follow-up between teams. FormAssembly allows us to create customized forms and workflows that collect the right information upfront, guide users through different paths based on their responses, and automatically send the data into Salesforce or Google Sheets.

We use it for a wide variety of needs, including customer-facing applications, internal request forms, surveys, event tracking, discount and financial assistance processes, and operational data collection. The Salesforce integration has significantly reduced duplicate entry and made it easier to create or update records, cases, and related information without requiring employees to manually transfer submissions. The Google Sheets integration gives other teams access to real-time results, which reduces reporting delays and eliminates the need to continually export and redistribute data.

FormAssembly has also allowed us to build solutions internally instead of relying on custom development for every new business need. This helps us launch processes more quickly, make updates as requirements change, improve data accuracy, and save teams substantial time on administrative work and manual handoffs.

**What do you like best about FormAssembly?**

Form Assembly is incredibly intuitive on the form building side. You can see exactly what your audience will see as you build it. The connectivity between forms and our software platform has increased efficiency across our organization significantly and decreased the need for tedious, manual data-entry tasks.

**What do you dislike about FormAssembly?**

The only possible negative is that learning how to use the pre-fill and post-survey connectors can be challenging. That said, there are numerous templates that can help with this, along with support tutorials and regular office hours offered by customer service. An AI chat bot is also available to support with error codes or general issues.

**What problems is FormAssembly solving and how is that benefiting you?**

Data entry used to take up significant time and effort across the organization. Now, forms are immediately uploaded, staff members are immediately notified, and the flow and efficiency of work has increased. In addition, we used to use numerous spreadsheets and other tracking methods that are now obsolete with the integration of Form Assembly.

**What do you like best about FormAssembly?**

Form Assembly was easy to install, set-up and use. We were able to connect to our Salesforce instance easily and started building forms immediately. Very user friendly while building forms and the options for question types, conditional visibility, and response types were exactly what we needed.

**What do you dislike about FormAssembly?**

Not many things to dislike about form assembly. The more branching logic you employed, the more tricky the user interface for form managers would become.

**What problems is FormAssembly solving and how is that benefiting you?**

It allows us to customize forms on our site to collect leads. We are able to have multiple forms in action on multiple pages, sites, etc. We're able to use Form Assembly to segment form submitters based on the URL they submitted from, their source, etc. We use form assembly's reporting functionality to review form submissions for completeness and to evaluate which forms are performing best.

**What do you like best about FormAssembly?**

What I like best about FormAssembly is the flexibility and depth of its integrations with other tools in our tech stack, especially Salesforce. The ability to build highly specific connectors allows us to design sophisticated workflows–for example, updating a particular Salesforce field in a precise way based on a respondent’s submission. Pre-fill connectors are especially valuable because they allow respondents to see information they have previously provided, confirm that we have it on file, and make updates when needed. This not only creates a smoother user experience but also helps build trust with our respondents by demonstrating that we are managing their data thoughtfully and accurately.

I’ve also consistently had positive experiences with FormAssembly’s support team. When questions or issues arise, their support has been responsive, knowledgeable, and helpful in working toward a solution.

**What do you dislike about FormAssembly?**

FormAssembly continues to improve, and we’ve benefited from many of the features added over time. That said, there are still a few relatively simple capabilities that would make form management and collaboration much easier. I’d especially like to see the ability to export a form, add internal comments or notes to individual fields or form elements, and generally have more tools for collaborating with colleagues during form development and review.

I also wish administrators were granted manager-level access to all forms by default. In our environment, this would significantly streamline internal workflows and troubleshooting. A model more similar to Salesforce–where system administrators can view and manage all records and configurations by default–would be very helpful.

Finally, I’d like to see more reusable functionality for connectors. FormAssembly allows us to add questions and sections to predefined content, but I wish we could do something similar with connector configurations. Being able to create and reuse standardized connector components would save time, reduce duplication, and make it easier to maintain consistency across forms.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is our primary data collection platform, and it plays an important role in helping us collect clean, consistent, and usable data at the point of entry. In-form validation helps prevent common data quality issues before information is submitted, while the flexibility and depth of FormAssembly’s Salesforce connectors allow us to control exactly how that information is written back to our system of record.

For my department, this significantly reduces the amount of manual data cleaning and troubleshooting required after collection. It gives us greater confidence that the data we rely on to monitor program administration, evaluate outcomes, and report on impact is accurate and analysis-ready. That allows our team to spend less time fixing data issues and more time using the data to generate insights and support decision-making across the organization.

**What do you like best about FormAssembly?**

FormAssembly provides a lot of flexibility with a relatively straightforward development environment. We use the forms for a variety of purposes, including general data gathering, higher education applications, enrollment and class registration, license renewals, etc. In most cases, forms are esigned connect/update Salesforce and also our credit card processor (Authorize.net). Also, the company seems focused on enhancements and growing the product. Recently, we started using the iATS/Deluxe connector to allow for donations from our  USA-based donors via ACH.  Also, now using the HTTPS connector, to pass data to Zapier for other internal workflows

**What do you dislike about FormAssembly?**

Support had been average at best, but over that past couple months response times have been lowered. The documentation is a bit clumsy to use, and there are features/steps undocumented. Some service reps know the product and can help resolve an issue timely, while others struggle to understand. In other words, support can be hit or miss.

**What problems is FormAssembly solving and how is that benefiting you?**

With FA we can quickly create forms to eliminate the need for fillable PDFs, plus eliminate data entry, since the forms connect to Salesforce.

**What do you like best about FormAssembly?**

From an admin perspective, FormAssembly set up is fairly straightforward. The user interface is good (maybe not as simple as some other form/survey tools but certainly more robust). The Salesforce connector (what our team uses is for most) is excellent and really easy to customize. FormAssembly also has tools that are explicitly HIPAA compliant, which is a non-negotiable for us. Overall, an excellent tool in a space where not many exist.

**What do you dislike about FormAssembly?**

Because FormAssembly is very robust and customizable, the learning curve is higher than other survey tools. I would not want a basic user creating surveys with connections, and I would anticipate them meeting issues quickly.

**What problems is FormAssembly solving and how is that benefiting you?**

Primarily taking internal and external data and bringing it into our CRM in a clear, clean, customizable format.

**What do you like best about FormAssembly?**

Client services support/onboarding could not have been more exceptional. They are very sensitive to the time they have with you.  Being a new uses they were helpful to assist in planning next steps and pieces to work in between our sessions.

**What do you dislike about FormAssembly?**

At this time I would like to have a better way to pull reports related to form submissions. This could be a gap that I have in knowledge.

**What problems is FormAssembly solving and how is that benefiting you?**

We are working to get to paperless forms and applications.  Regarding our applications, the time saved and reduction in stress for our applicants suffering from TBI's and PTSD, has been resounding to date.  Utilizing connectors and workflows has reduced their face to face demand with providers.  This is a common challenge and results in avoidance and delay in submissions.

**What do you like best about FormAssembly?**

I like that we can use it for multiple aspects/facets of our nonprofit's services. We use it for surveys and events, helping teachers to book field trips, and getting usage data from our Culture Pass libraries, we use FA for it all!

**What do you dislike about FormAssembly?**

I wish it had a built-in connector to Salesforce and other CRMs which would make a lot of our work easier when trying to collate data into our database. I also wish that our old system was not getting phased out. It is going to cost us a lot over the next few months to get moved to the new system because of how we have it connected to SF via Zapier.

Capturing data consistently in an easy-to-read and easy-to-use format.

**What do you like best about FormAssembly?**

I use FormAssembly to securely collect data and integrate it into our Salesforce environment. I appreciate the prefill feature, which allows us to input data we already know, saving time for people filling out the forms. The integration with Salesforce is key and very helpful because information goes right into our system, streamlining the process for form fillers. It's excellent, and I would rate it a 10 out of 10 for recommendation.

**What do you dislike about FormAssembly?**

I find there is a learning curve with FormAssembly, which is expected but still noticeable. The page loads for building the connectors can be very slow. There's a specific page where we map the form data to the database in Salesforce, and it uses connectors. Unfortunately, the page that loads those connectors can be slow. Additionally, the formula builder requires using a specific syntax, and it takes a bit of time to become familiar with it.

FormAssembly lets us securely collect data, prefill forms with known data, and integrate seamlessly with Salesforce. It streamlines data entry for users and sends information directly into our system, making the process efficient.

**What do you like best about FormAssembly?**

I use FormAssembly for integration and form management, which is crucial for my work at a small nonprofit. The platform's point-and-click interface makes it easier for me to manage these tasks without needing an external developer. I also appreciate the customization options which allow for some flexibility in how we use the tool.

**What do you dislike about FormAssembly?**

I find it challenging to work with FormAssembly, and the support has been unreliable. The UI for 'Build' is tedious to work and troubleshoot within. There were times when I reached out to support and had a support case open for months without hearing back, which felt unprofessional. I wish they offered timely, reliable support. There is a need for a more intuitive UI and helpful troubleshooting features. I believe FormAssembly has a lot of potential, but refinements are needed to achieve that potential.

FormAssembly connects Salesforce, Stripe, and our website, acting as an integration hub. It allows us to manage forms without an external developer. While it's okay for simple use cases, complex forms are challenging to configure and troubleshoot, and support needs improvement.

**What do you like best about FormAssembly?**

I really appreciate the user-friendly and powerful connectors in FormAssembly. They're easy to use, especially for basic connectors and logic. Embedding FormAssembly forms into an Experience Cloud site is incredibly helpful, and we use this functionality constantly. It allows students to use their Experience Cloud as a one-stop-shop for everything, including all the forms we house in FormAssembly.

**What do you dislike about FormAssembly?**

When it glitches, it's impossible to work with, and the customer support is basically nonexistent. For example, I have a CSV export that I've been waiting on for over 24 hours that's just stuck at 99%, and others that didn't get past 0% for over 3 hours that I abandoned. It's really finicky with browser cache/cookies—sometimes dropdowns or search fields just won't work because a user has some stale cookies on their browser session, and asking each user to reset their browser each time this happens is a huge pain. The Google Sheets connector only works about 45% of the time anyway.

**What problems is FormAssembly solving and how is that benefiting you?**

We use FormAssembly to streamline student data collection and programmatic tasks, keeping Salesforce as our source of truth. It ensures data consistency with standardized prefills and CRWE permissions.

**What do you like best about FormAssembly?**

We've been able to leverage form assembly instead of a net new experience cloud based build in Salesforce for student submitted forms which generate case and recommender records in Salesforce.  FormAssembly last year provided training to become FormAssembly certified which encompasses building flows and configurations within FormAssembly.

**What do you dislike about FormAssembly?**

Not too many things - More hands on based trainings (monthly?) to onboard new system users outside of the annual conferences.

**Recommendations to others considering FormAssembly:**

If you are a larger company with multiple teams, consider enterprise instead of having individual accounts to conserve money.  Also have team members join the VIP program to learn about new updates provided early to users in this program.

**What problems is FormAssembly solving and how is that benefiting you?**

We leverage form assembly for submission of support tickets which is sent to Salesforce for case creation as well as a way to allow student application recommenders to submit recommendations which are written to the student's salesforce opportunity record.

I like the introduction of drag and drop capabilities in matters form builder and this helps us in updating forms in real time
I acknowledge the importance of compliance and security features, which maintains reliability in handling sensitive data 
The process of data collection keeps improving, where we get valuable content from diverse sources.

I found FormAssembly quite inflexible, and this makes users get bored with some designs 
The program limits the reporting capabilities and this makes businesses less responsive

**What do you like best about FormAssembly?**

The best feature would be the new Ai functionality which allows me to upload a pdf file and the system quickly converts it into an inquiry form.

**What do you dislike about FormAssembly?**

Complex conditional logic, Salesforce connectors, and advanced workflows can require technical expertise or even some coding. This forces the common user to either seek training or contract experts.

**What problems is FormAssembly solving and how is that benefiting you?**

It reduces or eliminates data entry as it sends information to our CRM. Additionally, it replaces the paper process which streamlines processes and enhances the student experience.

FormAssembly is very user-friendly, and you can build a form with little to no training needed.

**What do you dislike about FormAssembly?**

When using workflows, you always have to build the form first. It would be better if the form creation were integrated into the workflow builder, so you could build the form as part of setting up the workflow.

FormAssembly has added AI, and it makes it much easier for me to complete my tasks.

Our account representative is extremely helpful and they are always working to improve and expand their offerings

There are more limitations to the functionality than I would prefer and many things that see like they are 'basic' functions are an added cost.

They are helping us make accessible forms that work for all of our audiences and are easily embedded in locations convenient to them

The Salesforce connectors and the conditional logics.

**What do you dislike about FormAssembly?**

The UI/UX could be better overall, and particularly when it comes to the look and feel of the forms, how they’re filled out, and the CSS. Also, it’s not a cheap tool.

**What problems is FormAssembly solving and how is that benefiting you?**

It helps us to close the GAP between the data we own and our automations. We use it a lot to conduct events invitations and lead generation.

**What do you like best about FormAssembly?**

The ability to create custom workflows and form handoffs is huge.  This has allowed us to create a smooth process for employee reimbursement requests from initial submission, to approval, to disbursement.

**What do you dislike about FormAssembly?**

I hope the certification program continues as that was very helpful in learning the system.

**Recommendations to others considering FormAssembly:**

This is a great product, but I highly recommend going through the certification process and learning the tools and features before fully implementing the forms.

**What problems is FormAssembly solving and how is that benefiting you?**

We use Form Assembly to collect valuable internal information and surveys, accept credit card payments, process program inquiries, and we are working on using workflow to build out multi user approval processes.

What I like the most about Form Assembly is how user friendly it is, while still allowing customisation to fit our business needs. I also appreciate that new features are added regularly, and their support team is absolutely amazing. The integration with Salesforce, which we use the most, is reliable and works seamlessly. Form Assembly is basically plug and play product, implementation took no time, as soon as we had log we immediately jumped in creating first form.

**What do you dislike about FormAssembly?**

There are only a few things I'd improve. Some of more advanced features often come at additional cost, and it sometimes feels like the ones we need most are at the higher-tier plans. For standard fields and connections it works perfectly but when working with repeaters or more complex setups things can get a bit tricky and may even require some coding. It would also be great to have more options for field types especially with date and time fields, which can get challenging to manage at times.

FormAssembly is reallt easy to use. I like how I can quickly create forms without writing any backend code. The AI agent is a greate feature, Like I just tell it what kind of form I need, like a contact or enquiry form and it builds it for me within seconds.

Sometime the UI feels a bit heavy when working with large forms and loading can take a few extra secs. But overall, it still works smoothly.

I really appreciate how polished the final forms look—they’re clean, professional, and easy to navigate for end users. It’s also very helpful that we can add hints and links to guide respondents, which improves the overall user experience.

There are a few limitations that can be frustrating. For example, it’s not possible to customize the confirmation email or the PDF we receive from the customer, which limits flexibility and branding options. Also, when creating many similar or repetitive forms, the platform can start to feel bulky and less efficient to manage. Another issue is the occasional loss of connection for no clear reason, which interrupts the workflow and can be a bit unsettling when working on a form.

I use FormAssembly in my job on a daily basis. 
Salesforce integration is straight forward and easy to use. If there are errors, it is clear to see where the integration failure was for an easy fix. 
The AI generated feature is easy when creating choices. 
Easy to navigate the different fields when building out forms. 
The ability to clone forms without losing their formatting is a huge plus (other platforms tend to lose some of the formatting or change the fields)
Customer support is great and they get back to you in a timely manner

**What do you dislike about FormAssembly?**

More opportunity for custom CSS (for example, changing the font of a form without compromising the layout of it)
I hope that FormAssembly will update their notification emails to respondents to allow a PDF attachment, rather than just the text in an email confirmation. That is the only downside of FormAssembly that other platforms have (e.g. Formstack).

I’m impressed with the simplicity of the UI. I can also use HTML codes in the forms to make them more customizable than they already are. I must also mention that the employees are incredibly friendly and welcoming. I’ve had the opportunity to meet a few people beyond our Account Representative (Beth Miller), and they’ve all been exceptionally pleasant and approachable. I want to give a special shoutout to Beth Miller for her exceptional job performance.

I always say it’s not realistic if you can’t mention a downside or two—if everything is perfect, it feels like you’re not being honest. But with FormAssembly, I really have to push myself to find something I dislike. If I had to point out one thing, it could be that the interface, while functional, feels a bit dated compared to some modern form builders. Also, while the platform is highly customizable, certain advanced configurations require more technical knowledge, which might be a challenge for users without a technical background. That said, these are minor issues in an otherwise excellent product.

The Salesforce connectors are excellent - having the picklist lookup connectors to Salesforce to automatically bring in active Salesforce values without us having to update multiple forms is such a timesaver, and gives us much more accurate data. We've also had excellent experience with payment connectors, and are looking forward to the improved Google Sheet connector coming soon. Workflow has improved vastly over the past year, and is making us jump to revamp old forms to better utilize its power. Using javascript to extend the capabilities is also excellent, as is the SAML prefill connector for SSO attributes. Overall it is intuitive and powerful, two things that don't often go together in software!

It is expensive, but you definitely get what you pay for!

FormAssembly has the most powerful Salesforce integration tools on the market. It is amazing how much work can be accomplished within the form to save time from creating and updating Salesforce records. The ability to handle multiple processes when the form is submitted is unmatched by any other form tool.

The customer facing survey very basic. Many other form tools have added varying ways for the customer to engage with the form, but FormAssembly only has the basic text, picklist, multiple choice options. It would be nice to make the form more engaging for the customers.

Using FormAssembly significantly reduces the amount of data entry required by staff by putting the customer in charge of entering their own data on a form. We then push all the form data to Salesforce using the integration tools FormAssembly has built.

It's very straightforward to build out a form. You can just drop in your different types of questions/fields and quickly edit and rearrange them within the editor.

We utilize a significant amount of custom CSS for our forms to match the style of our website, but there's no way to save universal CSS currently. You can create and save themes, but those only include basic settings around colors and fonts and whatnot. The CSS we utilize has to be copied and pasted into each form, and if we make a change once, we have to go back and update all of the other forms.

We host forms on our website, many of which are connected back to Salesforce, and FormAssembly allows us to automate many of the steps involved in getting data into and updated within Salesforce.

I love how easy FormAssembly is to use. You can quickly produce very sophisticated forms to cover a multitude of business needs. If you cannot figure out how to do something, FormAssembly offers an elaborate documentation library. The team at FormAssembly from sales to support has been extremely helpful and knowledgable in our experience.

The calcualted fields in forms are very useful, but are tough if you, like me, do not have expereience in JavaScript. When you are able to figure it out, its great and very powerful. But getting there can be difficult.

FormAssembly can be used for a number of purposes. We use it for event RSVPs, internal surveys, and as part of our grant processes. We connect it to our Salesforce org to maximize its functionality and maximize our data. Forms are extremely easy to set up and get running. Integration can be a little tricky, but in my opinion it's easier and has better UI than Form Titan. Customer Support has been responsive and helpful.

The licensing and permissions aspect is a little frustrating and difficult to understand/explain.

Maintenance of forms is a bit challenging.  It's a bit of a hassle to reconfigure to use a Salesforce sandbox.

We leverage FormAssembly to allow volunteers to sign up for shifts.  It looks up the volunteer in Salesforce (creates a new account if necessary)  and attachs a vounteer record to that volunteer.  Over time, we have expanded it's use to allow for volunteer groups to sign up, along with special types of vounteer events.  We also leverage the conditional logic parts to branch as necessary.  For example, when a volunteer is a minor, we check if they have parental permission on file, and request the parent contact information if not.

**What do you like best about FormAssembly?**

Overall formassembly is a great product to be able produce and maintain forms for salesforce environments. You can customize the look and feel of the forms to your organizations needs.

**What do you dislike about FormAssembly?**

That the connectors from Salesforce where easier to maintain and update. That there was a better way to make the connectors more secure

**What problems is FormAssembly solving and how is that benefiting you?**

It's solving the need of customer facing forms that are needed to be filled out for my clients organization.
