It reads the 835 remittance, ranks every denial by dollars and days left to file, and drafts the appeal letter with a citation to the CMS policy it relies on. A person at the billing company approves every letter before it goes out, and cases over $5,000 go to a manager. Billing companies pay $8 per appeal drafted ($6 above 1,000 a month, $4 above 5,000), with no seat licences, no minimum and no platform fee.
