**What do you like best about Unite Us?**

Unite Us is easy to use and straightforward.  Aside from it being a useful tool for us to keep track of our clients needs, it also assists in streamlining the referral process. This makes the responsibilities of our day to day more efficient and effective. I use the platform on a daily basis with such ease.  It is also very helpful for tracking purposes when we have to submit our interactions to our higher ups with clients. I use our calendar to reference how many appointments we've had and in turn use the platform for the details of the interactions for it to be more precise. Lastly, the few times I have run into issue using the platform, I have reached out to customer support and they have been very helpful.

Sometimes finding an outside organization to refer the client to can be convoluted.

**What do you like best about Unite Us?**

The best thing about Unite Us is that its platform is an all-in-one hub that provides information, resources, and direct referrals for health coordinators. Health Coordinators can readily see status of referrals and number of referrals made. Information that Health Coordinators would need that we often need to take extra time looking for is available in one place.

**What do you dislike about Unite Us?**

Unite Us does take time to understand and use. It is the type of platform that needs to be used regularly for it to become "easy." I also dislike that the websites are not checked to see if they are working. I have clicked on several websites that do not work.

**What problems is Unite Us solving and how is that benefiting you?**

Unite Us solves the problem of having to access multiple search platforms for resources that our members need access to. It also solves the problem of wondering how to make a referral. This benefits me as a health coordinator because I do not have to spend extra time calling programs and asking what their eligibility requirements are and how to make a referral. This also eleminates the question on if the referral was received and what the status of the referral is.

**What do you like best about Unite Us?**

Unite Us has helped us to build a network of over 90 advocacy organizations in the rare disease community. We are able to refer families to each other, clinicians are able to refer to us with one unique QR code, and together we can get families the support they need to thrive. The platform is easy to use and very user friendly for even the most tech-challenged individuals. Our account manager provides excellent guidance and support as each of our advocacy organization onboards to the Unite Us platform. The data insights dashboard gives us information about how we can do better, where we are succeeding, and critical information that allows us to evaluate our programs and what we need to create in order to help more families.

**What do you dislike about Unite Us?**

I would love if more people would join the platform and utilize the platform so that we can refer only on teh platform. While Unite Us has over 25,000 social service organizations, it would be great if even more organizations joined for ease of referring clients.

**What problems is Unite Us solving and how is that benefiting you?**

Unite Us connects patients with neurologic conditions to social service needs. We know that families who have children with neurologic conditions struggle more financially which leads to a host of other needs. We are able to quickly and efficiently find organizations to help our families and keep track of whether the referral was fulfilled through the close-looped referral system. No family ever gets left behind because we can always track progress made on each referral.

Sometimes navigating the various cards in the system can feel overwhelming, but David is always willing to help us.

**What do you like best about Unite Us?**

Unite Us has been implemented into Veterans Services of the Carolinas internal processes since inception. Unite Us, and the data analytics, have a allowed VSC to grow from a small operation in Asheville NC, to a state wide agency that serves as a model on the national stage.

**What do you dislike about Unite Us?**

The only downside I can see to using the system is the process of ensuring agency adoption and ensuring you have a robust network. Without engaged partners who all aknowledge that a single system ensures that best outcomes for clients, the platform can only be used to track internal cases.

**What problems is Unite Us solving and how is that benefiting you?**

Unite Us allows us to assist clients and their families in a holistic manner, addressing needs that can be handled internally or sent to another network provider.

Working with Unite Us has been a wonderful experience. The assigned team members were incredibly helpful and enthusiastic, going above and beyond to ensure our implementation and use was successful with our team members. They truly listened to the needs we expressed and worked diligently to deliver results within a realistic timeframe.

Not only was the service exceptional, but the platform itself has been a game-changer. It provides clear and insightful metrics that allow us to see the impact of our reach with clients. These metrics are invaluable for understanding our clients needs as well as gaps in the community.

Overall, this was an outstanding partnership, and we’re thrilled with both the service and the results. Highly recommend!

**What do you dislike about Unite Us?**

The biggest downside is the challenge of getting other partners fully on board with the platform. While it's so exciting to think about and see how this system can work successfully when the loop is closed, it can be frustrating for team members when other partners aren't fully engaged or using the platform.

The business problem being solved by using Unite Us is the inefficiency in connecting clients to necessary resources. Unite Us allows the team to not only document client interactions but also seamlessly connect them to resources when partners are on the platform. This has significantly reduced the time and effort it previously too to "chase down" referrals, streamlining the process and improving overall efficiency.

**What do you like best about Unite Us?**

Being able to find resouces outside of our footprint that can help our patients at SAI. Often times the patients we have are from far away, and we aren't familiar with their area they're from and what's available to them. But with a little bit of leg work we can get them the assistance they need and they can then focus on their own recovery.

**What do you dislike about Unite Us?**

Occasionally there are computer issues where it is difficult to get logged on. Last week it took me two hours just to complete a request for a patient which normally should only take a matter of minutes. We've called our IT team before and they have veriefied it was an issue with Unite US and not SAI.

**What problems is Unite Us solving and how is that benefiting you?**

United Us alllows us to match patients in our hospital with services in their area. Almost 100% of the time the patients here are not even aware these programs exist or are even available to them. By assisting them with this we can take these worries off of them so they can focus on getting well and the rehab they need to complete.

**What do you like best about Unite Us?**

As a statewide 211 entity, we appreciate having a data sharing agreement with Unite Us; hosting a Coordination Center with a focus on closed-loop referrals; serving as a front-door for coordinated entry housing programs; and most recently, we launched screening for Health Realted Social Needs benefits for the state of Oregon, where we can send and receive referrals on behalf of Medicaid members. We use Unite Us daily, across multiple programs, and can also store and share documents on behalf of clients. Integration of the platform alongside legacy systems has been easy to navigate and learn. Customer support is widely available when there are any issues that arrise.

**What do you dislike about Unite Us?**

This is more of a systems issue, but when an agency isn't using the platform, our clients have to reach out on their own without having that warm handoff.

Using the platform as a one-stop-shop to store client information, documents, and sending and receiving referrals on their behalf allows us to seemlessly focus on work to help millions of community members around our service region.

Unite Us provides an easy to use tool to connect community residents to local resources. Community Health Workers use it often to locate resources and suggests options they may have not thought of. When we run into any issues or questions, the Unite Us representative is always available to help implement new processes that are easy to understand.

**What do you dislike about Unite Us?**

Though we find Unite Us as our go to resource guide and connector, we are still learning how to create data sets that fit our needs. Having data sets that are easily mangeable would be helpful so we can create queries that are useful for our needs would be helpful.

Unite us is helping us solve the problem of connecting people to resources locally. This has helped us create healthier communities.

UniteUs makes sending and recieving referrals between partner agencies simple.  If you need to know what partners provide a particular service near a client the location search is a great feature.  UniteUs has a robust Tableau platform which makes understanding your service history and running reports easy.

The platform is purly a referral program with limited case management capabilities.

The UniteUs Platform provides a simple yet effective way for our organization to recieve, triage and connect clients with the appropriate partners to assist them with solving the needs that they have.

The Unite Us platform is user friendly, easy to navigate.  The platform make it simple to locate and refer individuals to needed services.  The data analytics which provide needed feedback are an added benefit to ensure services are being delivered and expected outcomes are achieved.

The platfom needs to be available to more of our team members.  Our agency of over 900 positions is not able to fund access for all our team members who beneift from the platform.

Unite Us resolves the issue of a closed loop referral system to ensure service that are referred are actually delivered.   Individuals referrals can be lost or receive a delayed response.  Unit Us assists in ensuring inviduals actually get connected to needed services and supports.

Unite Us is a great social connector that allows us to serve our citizens in a meaningful way and takes place of many manual processes and enhances the community!

**What do you dislike about Unite Us?**

It's not really a dislike, but enhancements to reporting would be useful as a power user that depends on theproduct..

**What problems is Unite Us solving and how is that benefiting you?**

Unite Us closes the the open end loop of manually referring individuals to other community organizations. The days of giving a flyer or phone number and hoping the client connects with them is over.   We can make a referral, see if the client engaged and facilitate the conversation.  It's amazing.  The Payments module has enchanced our ability to calculate and issue incentives to clients.

The application is user friendly and intuitive to use.  Users with limited technical experience have very few challenges when using Unite Us.  I have found the reporting applications to also be easy to use.  Customer support is timely and relevant, any questions I have had get answered.

**What do you like best about Unite Us?**

The platform interface is very user friendly allowing for reduced training times for new users. The support infastructure makes addressing things within the technology easy. All of our organizational processes flow through Unite Us and we have fully integrated it into all of our state and federally funded programs. Due to the user friendly interface, organizationa implimentation was easy, and customer success team provided all the support my organization needed.

**What do you dislike about Unite Us?**

Two biggest drawbacks for my personal use are:
1) No mobile app
2) Internal processes can be very segmented at times.

The technology has allowed us to assist over 20,000 people with SDoH needs across 21 service types as well as allowed us to pilot innovate upstream solutions to homelessness and suicide prevention.

**What do you like best about Unite Us?**

We moved to the Unite Us about a year ago, and have had to tailor the way we operate to match their database's abilities, but we have also had wonderful support from Unite Us staff, and I feel that our requests and suggestions are heard and acted on.  I also appreciate being invited to sit on Advisory Councils and other Unite Us meetings where we can express our opinions and learn more about the workings of the organization.

**What do you dislike about Unite Us?**

The least helpful think about Unite Us is not actually Unite Us' fault: we have community partners that we refer to heavily, and some of them have refused to move to the Unite Us platform, which means that we can't complete a closed loop referral to them.  We can do it outside of Unite Us, but it is much more difficult to remain in HIPAA compliance and to document the outside work for our grant reporting.  It's frustrating.

**What problems is Unite Us solving and how is that benefiting you?**

We complained to Unite Us that we weren't able to merge or delete duplicate records within the database.  Unite Us suggested a workaround that was ineffective for our needs; however, a few days later we got a call from the Unite Us IT department, asking if we would sit down with them and explain our situation with merging and/or deleting duplicate records.  I pulled my whole staff in to meet with the IT representative, and she graciously listened to our problems and showed us what Unite Us was working on for this problem, and then accepted our critique of how we thought it might be better implemented.  At the end of the call, she asked if we would be interested in having more of these sessions with them because our feedback had been so helpful.  We felt heard and we felt appreciated--and we are always available to give feedback.  That's the correct way to handle customer service complaints!

My organization just recently joined with Unite Us. Since joining, we have had nothing but positive, and caring employees to help our organization, and partners. Breshia and Beni have both been so patient with everyone we are bringing on to the platform, and truly made it feel like a 'small business' type of customer service. Besides great employees who are patient, kind, and willing to go out of their way to help us, the Unite Us platform itself has an amazing structure! I believe that adding Unite Us will take help our organization to grow and evolve. Thank you, Unite Us!

So far nothing, I am still navigating through the system, I would like to know if training is available so I can learn to use all the features.

Unite Us has been wonderful to partner with in order to connect our community resources. The platform is easy to use and there is a plethora of educational materials on how to utilize the platform. The data provided is invaluable to our community, it shows where the needs are and what services we still need. Everyone we have worked with on the Unite Us team has been friendly and more than willing to help. Using the Unite Us platform is helping to transform our community!

Any major change within a community takes time and reworking workflows for many organizations is difficult. I think time is the biggest barrier because we want the change now but the community organizations have to be ready for it.

**What problems is Unite Us solving and how is that benefiting you?**

Unite Us is solving the issue of disconnected resources. Many of our community organizations do not know that the other exsists or the full scope of the resources provided. Unite Us connects those dots with the community organizations so that the residents can be better served.

We have been fortunate to have an EXCELLENT account representative that has been very engaged and willing to meet with any of our stakeholders for communication about and dispersal of the technology, support, education, and network development. And, of course, what we like best is that we are getting our patients some help.

**What do you dislike about Unite Us?**

It's not a downside of Unite Us, but just the reality of these types of referrals - the more information you can enter into the referral, the more likely for it to be accepted and successful; thus, the referral takes a fair amount of time to enter.

Our data shows that patients with transportation, housing, food and other such needs are twice as likely to not get their care gaps closed, so we are hoping that connecting with these community services with help us deliver those services.

I am having difficulty logging in. I tried my usual login when prompted by an email and that didn't work. I tried logging in through my Wavnet portal and that didn't work and then I thought maybe I didn't register, so I registered and it brought me to an area to sign up for services.

The technology facilitates coordinating client services in one centralized system and the case management of mutual clients is very helpful. Easy to implemet and use the system and customer support via chat or help is always on point!  Our agency has easily integrated the Assistance Request Form which our stakeholders to frequently access our services.

What I do not like is that it is  not available on an app

By integrating the Assistance Request Form, this has taken the place of an Intake Form. We are also able to track referrals internally within our workforce system and network of partners that have joined the network.

**What do you like best about Unite Us?**

I appreciate many things about the platform.  It is user friendly and intuitive.   I like the dashboard and email notifications.  The ability to message other providers about cases is great.  The Insights reports have helped us and other agencies with tracking and applying for funding.  Having an Assistance Request form has been helpful to our agency.  The Customer Support team is timely and helpful.

**What do you dislike about Unite Us?**

I wish there was a free option for individuals to access the My Network Tab.  It would be great if there was a phone number listed for the sending agency within a referral.  Yes there's the messaging option, but sometimes it takes a while to hear back from and agency and a phone call is needed.

**What problems is Unite Us solving and how is that benefiting you?**

We used the Invoicing/Payer Work Que for an Housing Insecurities and Health Disparities grant that we had.  We utilize it for clients with critical medical needs to help stop a shut-off on electric and natural gas accounts.  I am the director of the Care Partner Network-an 3 state area collaboration that uses the UU platform for referring to each other.  We have served close to 1700 clients in the last 3 years.

Our partners found the ability to create and alter the programs and services they offer on their end, rather than our BPNC organization having to do it for them, and through customer support services in the past through NowPow.

We as a coalition are still interested in the ability to confirm client profiles via phone or email in addition to simply name and address confirmation. We feel these extra parameters would help us avoid duplicate accounts.

I can't think of anything specific, but we would be interested in creating and maintaining partner engagement with the software.

I work with families from many backgrounds. Some of their needs I am not as familiar with resources for. This tool is a great place for finding targeted resources quickly. It is easy to use and the creators are easy to contact/give feedback to.

I find that the process of sending resources to clients can be tedious. However, the features allow me to gather info and give it to families directly if they don't want to wait for a warm handoff.

The Unite Us team was incredibly helpful in our onboarding process. They were informative, imminentely available and flexible, and helpful. They have a clear planning and engagement strategy to work through every part of getting started. Once started, we were connected with someone locally who was well-informed and always available to help us problem-solve. I was truly blown away by their commitment to service. The features are excellent for care coordination and service engagement. They elicited feedback and took it under consideration through improvement tickets.

In the future, I think it would be amazing if families could also have access to Unite Us to build their own service coordination skillset and so they do not have to rely on a provider to learn about the resources available in their area.

Our competitive grant requires that we track the specific type, overall amount, and outcome of social determinants of health (SDoH) referrals; the Unite Us platform allows our statewide navigators to do this easily.
