We've been using 123 Form Builder for about 18 months for our agency as a solution for our client's need for HIPAA-compliant forms.
I can't say enough how pleased we are with the service. The platform is intuitive, secure, and reliable. Having the ability to create separate folders/sub-accounts for our clients and assigning users to those folders (so they can only see their contacts) has been key in implementing HIPAA-compliant forms across the board for our clients.
Additionally, the webhook integration has been key with integrating with our partner companies.
Our clients love the forms and use them for event registration, employment application, and more.

From time to time, during development, you may find the forms to be buggy. I have had difficulty in the past assigning a URL redirect for thank you pages when the 123FB team has released updates.

As a power user & administrator of 123FormBuilder, I can't recommend it highly enough. The SFDC integration is fully native and has lots of options for prefilling & mapping to existin & custom objects and fields. It's very easy to create forms. The user interface is good and is occasionally updated to make things easier. The integration with Salesforce and the method for prefilling and sending data is clear. Occasionally you encounter something that can't be done, but usually in these circumstances, there is a workaround or a different method that would work. The simplicity of creating the forms. Drag, drop, name, and that's it. The customizable output emails have led us to create many customer-facing forms that have our logos and pertinent data from their orders or accounts on them. We have twice as many forms that are used for internal process documentation and consistency. Our field engineers complete site survey forms, that submit output reports that go directly to our customers.
