SOAP Note AI Reviews & Product Details

SOAPNoteAI is an AI-powered clinical documentation tool (an AI medical scribe) that helps healthcare providers finish their notes in minutes instead of hours. Clinicians capture a visit in whatever way fits the encounter — record an in-person or telehealth session (up to two hours), dictate a quick verbal recap, upload an existing audio recording (up to three hours), or paste rough shorthand — and SOAPNoteAI generates a complete, properly structured SOAP note that is ready for review. Every plan is HIPAA-compliant. Data is encrypted in transit and at rest, audio recordings and transcriptions are automatically deleted within 24–48 hours, notes are never used to train AI models, and a Business Associate Agreement (BAA) is available. At the center of the product is Penny, an AI documentation assistant you can chat or talk to in order to edit, tighten, expand, or reassign any section of a note. Penny also generates treatment plans, referral letters, and patient visit summaries from your notes. An AI Clinician Review flags missing assessment elements before you finalize, and document versioning tracks every edit so you can restore any prior version. SOAPNoteAI supports multiple note formats — SOAP, DAP, BIRP — so it fits behavioral health as well as general medical documentation. It works across the web, a native iPhone and iPad app, and a Chrome extension that syncs all four SOAP sections directly into a SimplePractice appointment note in a single click (with universal one-tap copy for any other EHR). Built for solo clinicians and small practices with flat, transparent pricing — no per-seat enterprise contracts — SOAPNoteAI serves a wide range of specialties, from mental health, nursing, and physical therapy to chiropractic and veterinary medicine. New users get up to 20 free notes to start, with no credit card required. Trusted by 1,000+ providers.
