&lt;p&gt;Of the three factors, the audit trail is the one an examiner actually reads. Detection without a recorded disposition is arguably worse than no detection, because it creates evidence that the system flagged a call and no documented action followed. Transcription accuracy matters, but it feeds the same point: what a regulated team needs to show is flag, review, decision, remediation, in order, with timestamps. A platform with slightly weaker analytics and an airtight case-management trail is easier to defend than the reverse.&lt;/p&gt;
