Incident & Investigation lets anyone report an incident via a guided wizard — no account required — by QR code, anonymously or identified, from a mobile app that works offline; reviewers triage regulatory reportability with built-in notification timers (e.g., OSHA fatality within 8 hours, amputation within 24), and investigations run root-cause analysis with Fishbone, 5 Whys, and decision-tree tools. CAPA manages corrective and preventive actions from creation to verified completion. Risk maintains a central register with inherent and residual scoring and tracked controls. Inspections run on templates from a 23,000-strong pre-built library — or built in a Google-Forms-style builder, generated with AI, or converted from your existing forms — with recurring schedules, scored reports, and one-click CAPAs on findings. Beyond the core workflow: Assets & Maintenance tracks equipment records and maintenance history; Documents provides a versioned central store shared across every module; Training delivers a Coursera-style library completed inside the system with completion tracking and auto-reassign logic; and Reports are done-for-you — any regulatory or internal report built by the SDS Manager team, typically within a day. It took some time for our supervisors to figure out how detailed should they be, but the form does not provide any guidance on that matter.

There is only one record for incident, investigation and fix instead of splitting it into three different things.

**What problems is EHS Solution solving and how is that benefiting you?**

Reporting is straightforward in such a way that it can be done by anybody. Everybody is able to report anything via their phones without having an account, and something else goes through the process of examination before it reaches the destination. It is the examination process at two thousand employees that prevents the queue from being filled with noise.
