"Secure, Branded Surveys with Smart Logic That Keep Our Feedback Traceable"
4/5
What do you like best about Checkbox?

I love Checkbox because it’s built for secure, branded surveys and feedback collection, and I can tailor the survey logic to fit our dairy creamery’s quality feedback and stakeholder review process. We had a tough lesson with our raw milk supplier out of western Virginia earlier this quarter that showed how messy our old feedback system was. Before Checkbox, we sent out simple generic survey links through email for supplier quality reviews, post-delivery distributor feedback, and internal production crew safety input. All responses dumped into a basic spreadsheet, and there was no way to lock survey access or tie answers to specific batch numbers. That Virginia supplier got our quality feedback survey, and they replied that their milk test turnaround was going to slow down for three weeks. I only saw that note in the raw spreadsheet and forgot to flag it for our production team. Our production lead scheduled a full cheese run relying on their usual delivery timeline, and we ended up short on raw milk. We had to delay our order for a local grocery distributor and field a lot of frustrated messages from their buyer. That’s when I started looking for a dedicated survey tool that lets us control branding, data security, and tie feedback directly to our dairy operations.

What I like most is its smart survey logic and hosting flexibility. I can build separate surveys for supplier performance reviews, distributor product feedback, and internal post-batch quality check-ins. I added custom fields for batch ID, supplier lab certificate status, cold chain temperature logs, shelf-life feedback and distributor complaint tags. I can set the survey to skip irrelevant questions automatically. For example, if a distributor selects they had a temperature issue in transit, it pops up extra questions about the delivery window and storage handling. After that Virginia supplier incident, I moved all our recurring feedback surveys onto Checkbox. We host ours on the US cloud instance, which works well for our food safety data compliance requirements. I also white-label all the surveys with our creamery logo, so distributors and suppliers recognize it right away and are more willing to respond thoughtfully.

The CSV import is really handy when I need to pre-load contacts for our survey rounds. Every month, I pull our active milk suppliers and grocery distributors list, map all my custom dairy fields, and bulk upload them so each survey is pre-populated with their batch and order details. A few weeks ago, I loaded up our distributor list for our fall cheese launch feedback survey, and it saved me so much time instead of sending blank surveys that force buyers to type everything from scratch. Every survey response stays attached to the respondent’s record, and the full response history is easy to pull up. If our quality lead covers my work while I’m busy on the production floor, they can review all the supplier feedback without digging through old emails.

I also lean on the automated reminder feature. I set reminders to nudge suppliers and distributors who haven’t filled out their quality reviews by our cutoff date. Not long ago, a regional grocery buyer got our feedback survey about our yogurt line and hadn’t responded. The reminder went out on schedule, they filled it out and told us customers were asking for a smaller pack size. That feedback helped us plan our next production run. We’re a small dairy processor, and we didn’t want a bloated all-in-one platform. Checkbox works perfectly for us because it’s focused on secure, branded surveys with conditional logic. It hasn’t eliminated all manual follow-up, but it keeps our supplier and distributor feedback organized, traceable, and secure, so we don’t miss critical quality warnings that can disrupt our production schedules. What do you dislike about Checkbox?

Honestly, Checkbox is solid for sending out our supplier and distributor feedback surveys, but there are a few things that regularly slow me down, and I had a stressful run-in with that raw milk supplier out of western Virginia last month that drove it home. I was trying to bulk update survey contact records with their latest lab test status using a CSV upload ahead of our quarterly supplier review cycle. I matched all my column headers, double‑checked the custom fields for batch ID and test result notes, and uploaded the file. Most contacts updated fine, but that Virginia supplier’s feedback tag field got wiped blank. Our quality teammate didn’t see the missing note and sent them a standard performance survey, not the follow‑up survey we’d planned for their delayed test reports. They felt like we weren’t paying attention to their situation, and they almost held back their next milk shipment. Now I always save a backup and test with just two contacts first before uploading the whole roster, which adds extra prep time every quarter.

The built‑in reporting views are pretty limited for our dairy use case. My manager asked me to pull a report grouping all survey responses by supplier, batch number, cold‑chain feedback and complaint severity. Checkbox’s filters don’t stack well enough to get that all in one clean view. I have to export responses separately and cross‑reference everything in a spreadsheet. It takes most of my afternoon, and it’s easy to mix up batch IDs when I’m sorting dozens of distributor replies. I wish we could save custom survey report templates built around our food quality review cycles. Review collected by and hosted on G2.com.

"Helps Me Keep Health Surveys Organized and Easy to Manage"
4.5/5
What do you like best about Checkbox?

I work as a Dietician and Nutritionist, manage a homeopathy clinic, and also create health education content. I use surveys before nutrition consultations, health education sessions, and wellness programs. They help me understand eating habits, physical activity, sleep, stress levels, health goals, and other lifestyle information before I prepare personalized nutrition guidance. I also use surveys to collect feedback after workshops and awareness sessions, so having everything organized in one place is important for me. Checkbox fits well into this workflow. Creating surveys is simple, and I can use different question types depending on the information I need. If I want to update a question or make small changes later, I can do that easily without creating the survey again. Before sharing, I always preview the survey once to make sure everything looks right. I also like that I can keep different surveys organized and quickly find them again whenever I need to reuse or update them.

Sharing surveys is also straightforward. Sometimes I send a survey using a direct link, sometimes with a QR code. Once responses start coming in, I usually check the charts first because they give me a quick idea of common response patterns. If I need more details, I can review individual responses or export the data for my records and future planning. I use Checkbox regularly, and it has become part of my normal workflow. It was easy to implement, the number of features feels right for the kind of work I do, and I have never felt that the platform is complicated. Overall, it helps me collect better information, stay organized, and spend more time preparing nutrition guidance and health education content instead of managing survey responses manually. What do you dislike about Checkbox?

One thing I would like to see improved is the survey builder. It has all the options I need, but the overall layout feels a little crowded. A cleaner and more modern design would make building and editing surveys more comfortable, especially when working on longer forms. I also noticed that longer surveys sometimes take a little extra time to load in Preview, and the same can happen while opening detailed response reports so a little better performance would make the overall experience smoother. "Using Checkbox to help deliver services for schools, staff, and children all across Ireland"
5/5
What do you like best about Checkbox?

We use Checkbox as a portal for both the public and our employees to complete forms and collect essential data. As an administrator, I rely on Checkbox daily and truly value how intuitive and user-friendly it is. With more than 80 business units and 500 users in our department, managing multiple user groups and folders with the correct permissions is crucial, and Checkbox makes this process very straightforward. The reporting features are powerful and have greatly reduced the time we spend on data analysis. I especially appreciate the ability to add JavaScript action items, which lets us extensively customize our surveys when needed. The branching and conditional logic also enable complex forms to flow smoothly. Being able to use our own branding in the headers and URLs is very important to us. Checkbox’s support for distributing multilingual surveys is essential for our organization. Additionally, the Checkbox support team provides excellent documentation and is always extremely helpful and responsive whenever we require advanced assistance. Review collected by and hosted on G2.com.

What do you dislike about Checkbox?

Since we need to host Checkbox on-premise ourselves, managing the backend can sometimes be challenging. However, this setup is essential for data protection and provides us with reassurance, knowing that we have complete control over all the data we collect. Review collected by and hosted on G2.com.
