How best do we rinse and repeat on quarter over quarter programs?
It depends on your strategy and how you manage your budget. I recommend quarter to quarter campaigns, because you never know when the vendor might underperform and that budget is best spent with a different vendor.
If you're not changing anything about the campaign(s) i.e. the content, the targeting criteria, the targeted account lists, then I'd suggest working with your representative on defining how much budget and/or the lead volume you are ordering in a given timeline.
You can always ask for X amount of leads delivered in the quarter, but add preferences on the cadence of the lead flow (i.e. even lead delivery, front loading, etc.) .
