&lt;p&gt;On the before-or-after question: before is the right instinct, but it has a failure mode nobody mentions in demos. A pre-publish approval gate scales with human attention, and when volume grows, the queue does one of two things: becomes the bottleneck everyone resents, or degrades into rubber-stamping, where approval still technically happens but nobody is really looking. That second state is worse than no gate, because it looks like control. The setups that survive volume are tiered: automated rules clear the obviously fine and block the obviously bad, and humans only see the genuinely borderline. If a tool&#39;s approval workflow can&#39;t tier like that, it works right up until you succeed.&lt;/p&gt;
