&lt;p&gt;The failure I’d most want the platform to catch is access that survives a role change. Offboarding gets attention, but employees moving between teams can quietly retain entitlements they no longer need. I’d want automated certification to flag that excess access, show who approved or revoked it, and preserve the complete review history as audit evidence rather than relying on spreadsheets or manager recollection.&lt;/p&gt;
