&lt;p&gt;Most identity resolution and CDP tools aren&#39;t designed to sit on top of an EHR; connecting to Epic or Cerner is EMPI and integration-engine work. A general MDM layer can read from existing sources without full migration, but for clinical records, I&#39;d prioritize healthcare-native patient-matching vendors as complementary options.&lt;/p&gt;
