Hi Markley, you can create custom properties to add additional form fields. Have you tried this or are you looking for something like sliders? For example, for the surveys we send, we create a custom group of properties and then custom properties, then add those to the form. We aren't using anything but dropdowns and single-line text, however. Utilizing custom properties you can ask anything you like. :) (Gear/Properties)

This will show you how successful your social strategy is at driving traffic to your website, and how effective you are at converting that traffic into leads within the CRM.
