I have found Zarca Survey Software very useful for organizing surveys for my projects in a very easy way. It is a web based tool with integrated survey tool where you can set up various kinds of questions as well as different branching and data analysis options. Review collected by and hosted on G2.com.

Has complex branching logic or skip logic features which are suitable for desktop surveys are sometimes not suitable for mobile surveys.
