&lt;p&gt;The failure mode I&#39;ve heard about most often isn&#39;t the handoff itself breaking, it&#39;s that the context comes across but the human agent still starts from scratch anyway, either because they didn&#39;t read it or the summary missed the actual thing the person was frustrated about. So the technical handoff worked exactly as built, and the person still had to repeat themselves, just to a different party than expected. That suggests the harder problem isn&#39;t building the handoff, it&#39;s making sure the agent on the receiving end actually uses what got passed along.&lt;/p&gt;
