## FormAssembly Reviews
 ### 1. Great tool - doing better with support

**Rating:** 4.5/5.0 stars

**Reviewed by:** James P. | Director of Information Technology, Small-Business (50 or fewer emp.)

**Reviewed Date:** June 08, 2023

**What do you like best about FormAssembly?**

FormAssembly provides a lot of flexibility with a relatively straightforward development environment. We use the forms for a variety of purposes, including general data gathering, higher education applications, enrollment and class registration, license renewals, etc. In most cases, forms are esigned connect/update Salesforce and also our credit card processor (Authorize.net). Also, the company seems focused on enhancements and growing the product. Recently, we started using the iATS/Deluxe connector to allow for donations from our USA-based donors via ACH. Also, now using the HTTPS connector, to pass data to Zapier for other internal workflows

**What do you dislike about FormAssembly?**

Support had been average at best, but over that past couple months response times have been lowered. The documentation is a bit clumsy to use, and there are features/steps undocumented. Some service reps know the product and can help resolve an issue timely, while others struggle to understand. In other words, support can be hit or miss.

**What problems is FormAssembly solving and how is that benefiting you?**

With FA we can quickly create forms to eliminate the need for fillable PDFs, plus eliminate data entry, since the forms connect to Salesforce.

 ### 2. Exceptional Client Support and Onboarding Experience

**Rating:** 5.0/5.0 stars

**Reviewed by:** Amy M. | Applications Coordinator, Small-Business (50 or fewer emp.)

**Reviewed Date:** April 22, 2026

**What do you like best about FormAssembly?**

Client services support/onboarding could not have been more exceptional. They are very sensitive to the time they have with you. Being a new uses they were helpful to assist in planning next steps and pieces to work in between our sessions.

**What do you dislike about FormAssembly?**

At this time I would like to have a better way to pull reports related to form submissions. This could be a gap that I have in knowledge.

**What problems is FormAssembly solving and how is that benefiting you?**

We are working to get to paperless forms and applications. Regarding our applications, the time saved and reduction in stress for our applicants suffering from TBI's and PTSD, has been resounding to date. Utilizing connectors and workflows has reduced their face to face demand with providers. This is a common challenge and results in avoidance and delay in submissions.

 ### 3. Versatile Applications for all Nonprofits

**Rating:** 4.0/5.0 stars

**Reviewed by:** Anne O. | Communications and Culture Pass Manager at Act One, Small-Business (50 or fewer emp.)

**Reviewed Date:** February 02, 2023

**What do you like best about FormAssembly?**

I like that we can use it for multiple aspects/facets of our nonprofit's services. We use it for surveys and events, helping teachers to book field trips, and getting usage data from our Culture Pass libraries, we use FA for it all!

**What do you dislike about FormAssembly?**

I wish it had a built-in connector to Salesforce and other CRMs which would make a lot of our work easier when trying to collate data into our database. I also wish that our old system was not getting phased out. It is going to cost us a lot over the next few months to get moved to the new system because of how we have it connected to SF via Zapier.

**What problems is FormAssembly solving and how is that benefiting you?**

Capturing data consistently in an easy-to-read and easy-to-use format.

 ### 4. FormAssembly - Ease of integration and use w/ Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Verified User in Higher Education | Enterprise (> 1000 emp.)

**Reviewed Date:** May 19, 2022

**What do you like best about FormAssembly?**

We've been able to leverage form assembly instead of a net new experience cloud based build in Salesforce for student submitted forms which generate case and recommender records in Salesforce. FormAssembly last year provided training to become FormAssembly certified which encompasses building flows and configurations within FormAssembly.

**What do you dislike about FormAssembly?**

Not too many things - More hands on based trainings (monthly?) to onboard new system users outside of the annual conferences.

**Recommendations to others considering FormAssembly:**

If you are a larger company with multiple teams, consider enterprise instead of having individual accounts to conserve money. Also have team members join the VIP program to learn about new updates provided early to users in this program.

**What problems is FormAssembly solving and how is that benefiting you?**

We leverage form assembly for submission of support tickets which is sent to Salesforce for case creation as well as a way to allow student application recommenders to submit recommendations which are written to the student's salesforce opportunity record.

 ### 5. Powerful Salesforce Connectors and Flexible Conditional Logic

**Rating:** 4.5/5.0 stars

**Reviewed by:** Daniel A. | CRM & Marketing Automation Lead, Mid-Market (51-1000 emp.)

**Reviewed Date:** April 27, 2026

**What do you like best about FormAssembly?**

The Salesforce connectors and the conditional logics.

**What do you dislike about FormAssembly?**

The UI/UX could be better overall, and particularly when it comes to the look and feel of the forms, how they’re filled out, and the CSS. Also, it’s not a cheap tool.

**What problems is FormAssembly solving and how is that benefiting you?**

It helps us to close the GAP between the data we own and our automations. We use it a lot to conduct events invitations and lead generation.

 ### 6. Excellent features

**Rating:** 4.5/5.0 stars

**Reviewed by:** Verified User in Arts and Crafts | Small-Business (50 or fewer emp.)

**Reviewed Date:** May 19, 2022

**What do you like best about FormAssembly?**

The ability to create custom workflows and form handoffs is huge. This has allowed us to create a smooth process for employee reimbursement requests from initial submission, to approval, to disbursement.

**What do you dislike about FormAssembly?**

I hope the certification program continues as that was very helpful in learning the system.

**Recommendations to others considering FormAssembly:**

This is a great product, but I highly recommend going through the certification process and learning the tools and features before fully implementing the forms.

**What problems is FormAssembly solving and how is that benefiting you?**

We use Form Assembly to collect valuable internal information and surveys, accept credit card payments, process program inquiries, and we are working on using workflow to build out multi user approval processes.

 ### 7. Simple, Seamless Data Collection and CRM Integration

**Rating:** 5.0/5.0 stars

**Reviewed by:** Sai R. | Director of IT, Mid-Market (51-1000 emp.)

**Reviewed Date:** March 23, 2026

**What do you like best about FormAssembly?**

Formassembly makes it simple and straightforward to collect data and integrate it seamlessly with our CRM, without requiring complex application development or any coding.

**What do you dislike about FormAssembly?**

There isn’t much to say overall, except that I do run into a few glitches from time to time.

**What problems is FormAssembly solving and how is that benefiting you?**

Redeeming coupons, scheduling event registrations, and collecting information about participants in a program are all straightforward. I also like using prefilled forms with unique links, which makes it easier to gather the right details without extra back-and-forth.

 ### 8. Powerful, User-Friendly, Must-Have Platform

**Rating:** 5.0/5.0 stars

**Reviewed by:** Paula T. | Business Analyst and Salesforce Administrator, Mid-Market (51-1000 emp.)

**Reviewed Date:** October 21, 2025

**What do you like best about FormAssembly?**

What I like the most about Form Assembly is how user friendly it is, while still allowing customisation to fit our business needs. I also appreciate that new features are added regularly, and their support team is absolutely amazing. The integration with Salesforce, which we use the most, is reliable and works seamlessly. Form Assembly is basically plug and play product, implementation took no time, as soon as we had log we immediately jumped in creating first form.

**What do you dislike about FormAssembly?**

There are only a few things I'd improve. Some of more advanced features often come at additional cost, and it sometimes feels like the ones we need most are at the higher-tier plans. For standard fields and connections it works perfectly but when working with repeaters or more complex setups things can get a bit tricky and may even require some coding. It would also be great to have more options for field types especially with date and time fields, which can get challenging to manage at times.

**What problems is FormAssembly solving and how is that benefiting you?**

Form assembly helps us streamline data collection and automate workflows by connecting web forms directly with Salesforce. It eliminates manual data entry, reduces errors and ensures information flows smoothly between systems, saving time and keeping our processes efficient and consistent

 ### 9. Easy to Use with Seamless Form Connections Across Platforms

**Rating:** 5.0/5.0 stars

**Reviewed by:** Matthew S. | Data Specialist, Mid-Market (51-1000 emp.)

**Reviewed Date:** April 07, 2026

**What do you like best about FormAssembly?**

It's easy to use and connecting the forms to our different platforms is pretty seamless.

**What do you dislike about FormAssembly?**

Sometimes, I kind of have to play with the connecters to make sure they're connecting the correct SF fields.

**What problems is FormAssembly solving and how is that benefiting you?**

Collecting accurate data. This saves us time from having to correct what our users have filled out.

 ### 10. FormAssembly - Makes Form Creation and Data Collection Super Easy

**Rating:** 5.0/5.0 stars

**Reviewed by:** Kaushik M. | Frontend Team Lead | UI/UX Designer | Freelancer, Information Technology and Services, Small-Business (50 or fewer emp.)

**Reviewed Date:** October 18, 2025

**What do you like best about FormAssembly?**

FormAssembly is reallt easy to use. I like how I can quickly create forms without writing any backend code. The AI agent is a greate feature, Like I just tell it what kind of form I need, like a contact or enquiry form and it builds it for me within seconds.

**What do you dislike about FormAssembly?**

Sometime the UI feels a bit heavy when working with large forms and loading can take a few extra secs. But overall, it still works smoothly.

**What problems is FormAssembly solving and how is that benefiting you?**

I use FormAssembly for static websites that don't have a backend or admin panel. Earlier, I had to build a backend just to store form data, but now I can simply create forms here and view all the responses on the platform itself. It saves a lot of development time and helps me manage data easily.

 ### 11. Flexible Form Builder with Seamless Salesforce Integration, But Advanced Features Can Be Costly

**Rating:** 4.0/5.0 stars

**Reviewed by:** Verified User in Marketing and Advertising | Enterprise (> 1000 emp.)

**Reviewed Date:** November 10, 2025

**What do you like best about FormAssembly?**

What I like best about FormAssembly is how easy it makes creating and managing online forms that connect directly with Salesforce. As an account manager, I can quickly build customized forms for different client needs or campaigns without relying on IT support. The automatic syncing of responses into our CRM saves time and eliminates manual data entry, while the flexible design options ensure each form looks professional and on brand. It’s a dependable tool for collecting accurate information and improving the overall client experience.

**What do you dislike about FormAssembly?**

Some advanced features, such as like complex logic or deeper integrations, can require technical setup or higher-tier plans, which may limit flexibility for smaller teams.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is solving the challenge of efficiently collecting and managing client data from multiple sources. Before, gathering information through emails or spreadsheets was time-consuming and prone to errors. Now, everything flows through organized online forms that automatically sync with Salesforce, ensuring our records are accurate and up to date. This not only saves time but also improves data quality, speeds up follow-ups, and creates a smoother experience for both clients and internal teams.

 ### 12. Very Easy to Navigate with Helpful Drop-Down Menus

**Rating:** 4.0/5.0 stars

**Reviewed by:** Julia E. | Admissions and Records Officer, Small-Business (50 or fewer emp.)

**Reviewed Date:** April 09, 2026

**What do you like best about FormAssembly?**

Very easy to navigate, drop down menu helpful

**What do you dislike about FormAssembly?**

would be helpful to use the "enter" key instead of clicking the button

**What problems is FormAssembly solving and how is that benefiting you?**

Students submitting in one portal and not with email, which might get lost in outlook

 ### 13. Quite good

**Rating:** 4.0/5.0 stars

**Reviewed by:** Paulina P. | Global Payroll Implementation Specialist, Enterprise (> 1000 emp.)

**Reviewed Date:** May 29, 2025

**What do you like best about FormAssembly?**

FormAssembly is a user-friendly and intuitive tool for creating forms. The drag-and-drop interface makes building forms fast and straightforward, even for more complex needs. I really appreciate how polished the final forms look—they’re clean, professional, and easy to navigate for end users. It’s also very helpful that we can add hints and links to guide respondents, which improves the overall user experience.

**What do you dislike about FormAssembly?**

There are a few limitations that can be frustrating. For example, it’s not possible to customize the confirmation email or the PDF we receive from the customer, which limits flexibility and branding options. Also, when creating many similar or repetitive forms, the platform can start to feel bulky and less efficient to manage. Another issue is the occasional loss of connection for no clear reason, which interrupts the workflow and can be a bit unsettling when working on a form.

**What problems is FormAssembly solving and how is that benefiting you?**

We use this tool to check the profile and mandatory requirements of future clients

 ### 14. From Employee to User

**Rating:** 5.0/5.0 stars

**Reviewed by:** Steven W. | Systems Administrator, Small-Business (50 or fewer emp.)

**Reviewed Date:** October 07, 2025

**What do you like best about FormAssembly?**

Along with an easy to use product, the Sales and Support teams at FormAssembly have been and are always stellar to work with. They are helpful, timely, responsive, and always a delight.

**What do you dislike about FormAssembly?**

It is not free. I would have nothing to dislike if it were free, though. :)

**What problems is FormAssembly solving and how is that benefiting you?**

At Skeens Warehouse Services we have a large network of trusted partners and we onboard them and obtain important documents using FormAssembly. This makes sure our data is uniform and portable in anyway that we need it.

 ### 16. User friendly platform

**Rating:** 5.0/5.0 stars

**Reviewed by:** Sarah J. | Program Support Specialist, Small-Business (50 or fewer emp.)

**Reviewed Date:** May 23, 2024

**What do you like best about FormAssembly?**

I use FormAssembly in my job on a daily basis.
Salesforce integration is straight forward and easy to use. If there are errors, it is clear to see where the integration failure was for an easy fix.
The AI generated feature is easy when creating choices.
Easy to navigate the different fields when building out forms.
The ability to clone forms without losing their formatting is a huge plus (other platforms tend to lose some of the formatting or change the fields)
Customer support is great and they get back to you in a timely manner

**What do you dislike about FormAssembly?**

More opportunity for custom CSS (for example, changing the font of a form without compromising the layout of it)
I hope that FormAssembly will update their notification emails to respondents to allow a PDF attachment, rather than just the text in an email confirmation. That is the only downside of FormAssembly that other platforms have (e.g. Formstack).

**What problems is FormAssembly solving and how is that benefiting you?**

Salesforce integration - the responses submitted are efficiently being put into Salesforce for ease of reading the applications and connecting them to the appropriate accounts.

 ### 17. Build customized forms for any need. Connect to your 3rd party CRM like Salesforce.

 ### 19. Easy integration

**Rating:** 4.0/5.0 stars

**Reviewed by:** Amanda G. | Advocacy Center UniServ Director, Small-Business (50 or fewer emp.)

Workflows is the best aspect of Form Assembly. It is a streamlined process that allows users to connect forms. The customer service support is exceptional and offers explicit details through hands-on delivery.

**What do you dislike about FormAssembly?**

I wished there was a way that form responses could be used when customers need to make another draft of the same response,

**What problems is FormAssembly solving and how is that benefiting you?**

The addition of workflow has diminished time spent tracking multiple responses.

 ### 20. Great UI, Customization, and Exceptional Customer Service

**Rating:** 5.0/5.0 stars

**Reviewed by:** Ilker Colt D. | Salesforce Business Analyst, Enterprise (> 1000 emp.)

**Reviewed Date:** January 31, 2025

**What do you like best about FormAssembly?**

I’m impressed with the simplicity of the UI. I can also use HTML codes in the forms to make them more customizable than they already are. I must also mention that the employees are incredibly friendly and welcoming. I’ve had the opportunity to meet a few people beyond our Account Representative (Beth Miller), and they’ve all been exceptionally pleasant and approachable. I want to give a special shoutout to Beth Miller for her exceptional job performance.

**What do you dislike about FormAssembly?**

I always say it’s not realistic if you can’t mention a downside or two—if everything is perfect, it feels like you’re not being honest. But with FormAssembly, I really have to push myself to find something I dislike. If I had to point out one thing, it could be that the interface, while functional, feels a bit dated compared to some modern form builders. Also, while the platform is highly customizable, certain advanced configurations require more technical knowledge, which might be a challenge for users without a technical background. That said, these are minor issues in an otherwise excellent product.

**What problems is FormAssembly solving and how is that benefiting you?**

We have some users and clients who don’t have access to Salesforce. However, they still need to input data (such as cases or other types of records) for use. We are building forms with FormAssembly so that we can use it.

 ### 21. The best form builder for Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Moses K. | Salesforce Admin, Mid-Market (51-1000 emp.)

**Reviewed Date:** October 22, 2024

**What do you like best about FormAssembly?**

What I like best about FormAssembly is its comprehensive connectors that make integrating with standard or custom objects seamless. The platform's ability to work effectively with Salesforce integration user accounts is a huge plus, as it helps save costs by avoiding the need for standard licenses. Additionally, the customer support is excellent, and there is a rich, knowledgeable community that offers valuable resources and guidance.
I also like the fact that you can embed salesforce community forms with ease.
Indepth error logs
We currently depend on the platform daily operations and lead generation.
Easy to implement as long as one has admin permissions on both platforms.
This combination of powerful integration capabilities, cost-saving features, and strong support makes FormAssembly an outstanding choice for data collection and form management.

**What do you dislike about FormAssembly?**

More error handling options would make the platform even better such as error notification emails.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is solving the challenge of seamless data collection and integration with Salesforce. By allowing forms to connect directly to both standard and custom objects, it streamlines the process of capturing and managing data without the need for manual entry. This not only saves time but also reduces the risk of data errors. The platform's ability to utilize Salesforce integration user accounts helps significantly in saving costs, as it avoids the need for additional standard licenses. Furthermore, the rich support community and responsive customer service ensure that any issues are quickly resolved, enhancing productivity and efficiency.
- lead generation with limited duplicates
- application reviews
- multi step business processes
- duplicate handling

FormAssembly integrates well with Salesforce. It allows us to collect and bring data into the CRM. We can also prefill forms to ensure data quality.

**What do you dislike about FormAssembly?**

There is a learning curve with FormAssembly. The page loads for the connectors can be very slow.

**What problems is FormAssembly solving and how is that benefiting you?**

We use FormAssembly to collect data from external users and integrate the data with Salesforce. Support for pre-filled links streamlines the process.

 ### 25. FormAssembly continually improves our user experience with our systems

**Rating:** 5.0/5.0 stars

**Reviewed by:** Elizabeth Y. | Enterprise Applications Administrator/Developer, Mid-Market (51-1000 emp.)

**Reviewed Date:** September 13, 2024

**What do you like best about FormAssembly?**

The Salesforce connectors are excellent - having the picklist lookup connectors to Salesforce to automatically bring in active Salesforce values without us having to update multiple forms is such a timesaver, and gives us much more accurate data. We've also had excellent experience with payment connectors, and are looking forward to the improved Google Sheet connector coming soon. Workflow has improved vastly over the past year, and is making us jump to revamp old forms to better utilize its power. Using javascript to extend the capabilities is also excellent, as is the SAML prefill connector for SSO attributes. Overall it is intuitive and powerful, two things that don't often go together in software!

**What do you dislike about FormAssembly?**

It is expensive, but you definitely get what you pay for!

**What problems is FormAssembly solving and how is that benefiting you?**

From general forms to complex workflows with approvals from multiple parties (linear or asynchronous), from payment forms to event registration to contracts to secure views of Salesforce data for non-licensed users to application portals, this is our go-to.

 ### 26. Powerful Salesforce Integration

**Rating:** 5.0/5.0 stars

**Reviewed by:** Derick H. | Senior Director, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 27, 2024

**What do you like best about FormAssembly?**

FormAssembly has the most powerful Salesforce integration tools on the market. It is amazing how much work can be accomplished within the form to save time from creating and updating Salesforce records. The ability to handle multiple processes when the form is submitted is unmatched by any other form tool.

**What do you dislike about FormAssembly?**

The customer facing survey very basic. Many other form tools have added varying ways for the customer to engage with the form, but FormAssembly only has the basic text, picklist, multiple choice options. It would be nice to make the form more engaging for the customers.

**What problems is FormAssembly solving and how is that benefiting you?**

Using FormAssembly significantly reduces the amount of data entry required by staff by putting the customer in charge of entering their own data on a form. We then push all the form data to Salesforce using the integration tools FormAssembly has built.

 ### 27. A solid front-end form system with potential for greater things

**Rating:** 3.5/5.0 stars

**Reviewed by:** Sean Y. | Digital Communications Manager, Mid-Market (51-1000 emp.)

**Reviewed Date:** February 03, 2023

**What do you like best about FormAssembly?**

It's very straightforward to build out a form. You can just drop in your different types of questions/fields and quickly edit and rearrange them within the editor.

**What do you dislike about FormAssembly?**

We utilize a significant amount of custom CSS for our forms to match the style of our website, but there's no way to save universal CSS currently. You can create and save themes, but those only include basic settings around colors and fonts and whatnot. The CSS we utilize has to be copied and pasted into each form, and if we make a change once, we have to go back and update all of the other forms.

**What problems is FormAssembly solving and how is that benefiting you?**

We host forms on our website, many of which are connected back to Salesforce, and FormAssembly allows us to automate many of the steps involved in getting data into and updated within Salesforce.

 ### 28. FormAssembly: an easy to use, intuitive tool backed by a great staff

**Rating:** 5.0/5.0 stars

**Reviewed by:** Jared E. | Lead Salesforce Administrator, Small-Business (50 or fewer emp.)

**Reviewed Date:** August 27, 2024

**What do you like best about FormAssembly?**

I love how easy FormAssembly is to use. You can quickly produce very sophisticated forms to cover a multitude of business needs. If you cannot figure out how to do something, FormAssembly offers an elaborate documentation library. The team at FormAssembly from sales to support has been extremely helpful and knowledgable in our experience.

**What do you dislike about FormAssembly?**

The calcualted fields in forms are very useful, but are tough if you, like me, do not have expereience in JavaScript. When you are able to figure it out, its great and very powerful. But getting there can be difficult.

**What problems is FormAssembly solving and how is that benefiting you?**

Allows us to effeciently build forms that can be client or internal facing and read into Salesforce. This allow us to collect data that would otherwise need to be manually input into our org.

 ### 29. Multi-purpose tool

**Rating:** 4.5/5.0 stars

**Reviewed by:** Edison P. | Salesforce Administrator, Small-Business (50 or fewer emp.)

**Reviewed Date:** August 27, 2024

**What do you like best about FormAssembly?**

FormAssembly can be used for a number of purposes. We use it for event RSVPs, internal surveys, and as part of our grant processes. We connect it to our Salesforce org to maximize its functionality and maximize our data. Forms are extremely easy to set up and get running. Integration can be a little tricky, but in my opinion it's easier and has better UI than Form Titan. Customer Support has been responsive and helpful.

**What do you dislike about FormAssembly?**

The licensing and permissions aspect is a little frustrating and difficult to understand/explain.

**What problems is FormAssembly solving and how is that benefiting you?**

We implemented FormAssembly to replace Classy, a more expensive platform that could only do event registration. We're able to gather data internally and externally using FormAssembly. Data can drop into Salesforce with an integration.

 ### 30. Access to Salesforce for non-Salesforce users.

**Rating:** 4.5/5.0 stars

**Reviewed by:** Paul M. | CRM (Salesforce) Leader, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 27, 2024

**What do you like best about FormAssembly?**

It provides the ability for non-Salesforce users to access Salesforce data.

**What do you dislike about FormAssembly?**

Maintenance of forms is a bit challenging. It's a bit of a hassle to reconfigure to use a Salesforce sandbox.

**What problems is FormAssembly solving and how is that benefiting you?**

We leverage FormAssembly to allow volunteers to sign up for shifts. It looks up the volunteer in Salesforce (creates a new account if necessary) and attachs a vounteer record to that volunteer. Over time, we have expanded it's use to allow for volunteer groups to sign up, along with special types of vounteer events. We also leverage the conditional logic parts to branch as necessary. For example, when a volunteer is a minor, we check if they have parental permission on file, and request the parent contact information if not.

 ### 31. Good use of forms inside salesforce

**Rating:** 4.0/5.0 stars

**Reviewed by:** Ryan R. | Salesforce Consultant, Small-Business (50 or fewer emp.)

**Reviewed Date:** September 24, 2024

**What do you like best about FormAssembly?**

Overall formassembly is a great product to be able produce and maintain forms for salesforce environments. You can customize the look and feel of the forms to your organizations needs.

**What do you dislike about FormAssembly?**

That the connectors from Salesforce where easier to maintain and update. That there was a better way to make the connectors more secure

**What problems is FormAssembly solving and how is that benefiting you?**

It's solving the need of customer facing forms that are needed to be filled out for my clients organization.

 ### 32. Revolutionize and Automate Mundane Tasks Using FormAssembly!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Adam J. G. | Director of Sleep Services, Small-Business (50 or fewer emp.)

**What do you like best about FormAssembly?**

Revolutionize and Automate Mundane Tasks Using FormAssembly!

FormAssembly makes it easy to accomplish these goals. From a stellar interface to amazing customer support and everything in between. It was incredibly easy to implement into our workflow and brought us tremendous value in how we use it.

FormAssembly is used daily and many times per day utilizing patient facing questionnaires, evaluation of services, etc. In the future we plan to use it for patient scheduling. It was incredibly ease to integrate with our system.

**What do you dislike about FormAssembly?**

I haven't encountered any downsides yet.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly saves my staff from having to call patients on the phone, collect information, then spend time putting that data into our system. FormAssembly takes the data from the patient and brings it directly into our system providing invaluable time savings and value.

 ### 33. incredably easy to use with our Salesforce orgs

**Rating:** 5.0/5.0 stars

**Reviewed by:** Robert W. | CRM Solutions Manager, Mid-Market (51-1000 emp.)

**Reviewed Date:** December 05, 2017

**What do you like best about FormAssembly?**

FA just makes work so much easier to complete. I love:
* How easy it is to add form questions.
* The Prefill connectors are simple and straightforward
* branding/themes
8 the connector is so powerful and easy to use. I love that cross-function object connections make it so easy to upset records to multiple objects in Salesforce

**What do you dislike about FormAssembly?**

OMG why the hell are there no folder options? All the forms are just dumped into one big section. I have almost 100 forms and tags just don't do it for me the way organizing in folders would. Please for the love of God add folder options

**What problems is FormAssembly solving and how is that benefiting you?**

entering our leads from school and fair visits and being able to sync those leads to campaigns

 ### 34. Great tool for intaking data into Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Ryan L. | Manager, Enterprise Technology, Leisure, Travel & Tourism, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 27, 2024

**What do you like best about FormAssembly?**

Ease of use for our developers to stand up projects quickly, the tool allows us to build forms quickly without starting from scratch. Customer service with support has been good as well when the team has questions.

**What do you dislike about FormAssembly?**

Nothing to dislike about it, it's a great tool,

**What problems is FormAssembly solving and how is that benefiting you?**

It's helps us to stand up projects faster and easier than if we were to build forms from scratch each time, it simplifies the integration work and the UI development specifically.

 ### 35. Form Assembly is super user friendly and low/no code development! Fix the Bugs and it gets an A+!

**Rating:** 4.5/5.0 stars

**Reviewed by:** Verified User in Manufacturing | Enterprise (> 1000 emp.)

**Reviewed Date:** August 27, 2024

**What do you like best about FormAssembly?**

Form Assembly for the price is a really robust solution. Their data security is top notch, surveys and workflows are extremely powerful, and end user configuration is really quite easy to get the hang of to build and maintain with ease. With an implementation manager we were able to build out a fairly complex and robust workflow solution within Form Assembly successfully with limited support and empowering us, the customer, to own that product build out.

The tool continues to be enhanced with additional features. Requests have been submitted fro additional enahncements and their DEV team seems open to ideas, but slow to actual produce and develop. Their customer service team definitely provides exceptional support- most tickets are responded to same day within hours of their submission.
We do not have an integration with Form Assembly (were attempting to connect via an API into the workflow solution- but Form Assembly only has an API stood up for surveys, not workflow) so I cannot provide feedback from here.

This software is used daily by our company and has continued to provide efficiencies and benefits across all the users that leverage it. Way to go Form Assembly for providing a robust simple solution for our teams to leverage!

**What do you dislike about FormAssembly?**

We have asked for several development enhancements that we feel could be beneficial beyond just our use cases. Unfortunately none of our ehnahcement requests have been taken to the point of DEV enhancement or release.

We leverage workflows heavily within Form Assembly and although very dynamic and robust- periodically one crashes (less than 1% of the time) but when a workflow crashes for no apparent reason- there is no reason/trigger or notification... so it either has to be processed manually outside the system workflow or deleted out and restarted. Wish the glitch rate was 0% instead 0.5%.

Beyond these couple deficiencies Form Assembly is really quite good. Bang for your buck they do provide a very solid software solution.

**What problems is FormAssembly solving and how is that benefiting you?**

What used to be a labor intensive manual survey and workflow solution, Form Assembly provides a digital software solution to perform this in an automated digital manner.

 ### 36. Valuable data collection tool

**Rating:** 5.0/5.0 stars

**Reviewed by:** Mary M. | Salesforce Administrator, Mid-Market (51-1000 emp.)

**Reviewed Date:** July 18, 2024

**What do you like best about FormAssembly?**

I love that FormAssembly lets us collect data from people external to our Salesforce organization without creating duplicates or exposing vulnerable information. We also make great use of their SSO security and form prefill to make employee-centric forms both secure and accessible.

**What do you dislike about FormAssembly?**

FormAssembly's formula writing tool can be comparatively difficult to work with, and the Response queries/reports are sometimes a little janky. Many of FormAssembly's best features are locked behind higher-cost tiers, which can create barriers for nonprofits. Still, it's more than worth it.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is helping us capture secure data and feed it into Salesforce without having to buy everyone a Salesforce license. It's also cut down on our manual data entry immensely, saving us a ton of time and making our entire techstack more approachable to end users.

 ### 37. Digital web forms - Powerful Salesforce integration with workflows, Pre-fills & Payment Connector

**Rating:** 5.0/5.0 stars

**Reviewed by:** Brian W. | Salesforce Administrator, Mid-Market (51-1000 emp.)

**Reviewed Date:** May 23, 2024

**What do you like best about FormAssembly?**

How easy it is to create a form and map it to a salesforce data set. The Salesforce integration is my favorite part, it was very easy to set up. (I'm biased, I'm a Salesforce pro) But the customizations you can make on the form, and then how you can have those form entries trigger other salesforce automation was a game changer for our customer intake process. We also have web forms and a payment form where you can receive online payments with a web form! The customer support was always responsive and super helpful when exploring potential use cases. We use the form daily, and hope to increase the overall use.

**What do you dislike about FormAssembly?**

I can't think of any. I'm not sure if there would be any limitations on the UI or if you wanted to customize anything, but I don't have complaints.

**What problems is FormAssembly solving and how is that benefiting you?**

Capturing all customers who come into our three different locations. We were not doing this before, and now everyone is automatically entered as a new contact into Salesforce (if they're not already in). Formassembly can know if a contact is already in Salesforce, so it won't create duplicates if you set up the form and map it to salesforce correctly. We also easily setup a web payment form to collect online credit card payments, which is helping out back end team.

 ### 38. Form Assembly makes gathering information a breeze

**Rating:** 5.0/5.0 stars

**Reviewed by:** Leona B. | Systems and Data Coordinator, Mid-Market (51-1000 emp.)

**Reviewed Date:** August 14, 2023

**What do you like best about FormAssembly?**

I like that we can make each form unique and customizable to our events. We can change background images, colors, just about anything we want with a few simple clicks.

**What do you dislike about FormAssembly?**

I wish some of the connection pieces between the form and our CRM would save across forms. Something along the lines of a basic connection setup and then have the option to tweek it as needed would be wonderful!

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly gives us a hands-free approach to collecting lead and prospect information, as well as gather what is needed for event registrations. The fact that the data drops right into our CRM is priceless!

 ### 39. Easy to use; love the quick connections to Salesforce!

**Rating:** 5.0/5.0 stars

**Reviewed by:** Verified User in Higher Education | Enterprise (> 1000 emp.)

**Reviewed Date:** January 26, 2017

**What do you like best about FormAssembly?**

Ease of creating forms that connect to Salesforce
Ability to publish quickly

**What do you dislike about FormAssembly?**

Lack of capabilities to give "selection" option on Salesforce pre-fill connector - i.e. if I get back more than one record from my LOOKUP, want to have ability to view & select the appropriate record prior to submitting form.

**Recommendations to others considering FormAssembly:**

If you're looking for an easy-to-use form building tool that easily connects your forms to Salesforce, then this is the product for you!

**What problems is FormAssembly solving and how is that benefiting you?**

Have a single form building tool in our environment that is user-friendly and easily connects our forms to Salesforce.

 ### 40. Very excited for things to come.

**Rating:** 4.5/5.0 stars

**Reviewed by:** Albert S. | Associate Director of Admissions Systems, Higher Education, Enterprise (> 1000 emp.)

**Reviewed Date:** May 15, 2024

**What do you like best about FormAssembly?**

I have always appreciated the support team at FormAssembly. Beth, our account rep, is great to work with. The Dev team is always improving their product with new and improved features. I am very excited about what is coming. When I was assigned to learn this application, I found it easy to learn and use.

**What do you dislike about FormAssembly?**

Being on the legacy platform, the visible working space was hard to work with. But now after the revamp, the working area is looking great.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is helping us with Speed To Lead. Being integrated with Salesforce, we can acquire all the relevant data and start campaigns immediately.

 ### 41. Great Solution to Form Problem

Clunky builder makes it a little difficult to create forms.

 ### 42. The most powerful tool that I use day to day in Salesforce

**Rating:** 5.0/5.0 stars

**Reviewed by:** Jonathan P. | Small-Business (50 or fewer emp.)

**Reviewed Date:** January 23, 2024

**What do you like best about FormAssembly?**

Clear documentation, fast and friendly chat support, easy implementation and well documented updates. Regular additional training and update information available through the company. Does some real magic that we love to utilize the system for.

**What do you dislike about FormAssembly?**

Email support is difficult, especially compared to chat support (which is phenomenal). The tool is incredibly powerful, so it feels complicated at times. I feel at times that I want to carve out a day to work in FormAssembly, but my schedule doesn't allow the big blocks of time that are required for bigger efforts. It is hard to take a big effort in FA and carve it up into tiny pieces.

**What problems is FormAssembly solving and how is that benefiting you?**

Reduces the need for my employees to do data entry and gives them the power to do the auditing side of data entry.

**Reviewed Date:** August 30, 2024

**What do you like best about FormAssembly?**

The Salesforce connector is the most comprehensive and easy to use.

**What do you dislike about FormAssembly?**

I wish the styling of fields and templates was a bit more advanced within the platform itself. More Point and click for styling as opposed to code, that tends to be problematic when using REST API.

**What problems is FormAssembly solving and how is that benefiting you?**

Prefilled forms for information submission to pur product in Salesforce.

 ### 45. Great form automation and integration with Salesforce

Support is great as are the on-line help documents.

**What do you dislike about FormAssembly?**

At this time we have not found any showstoppers with any of our form designs.
Missing a Connector to NetSuite

**What problems is FormAssembly solving and how is that benefiting you?**

As a food bank we interact daily with the 500+ food pantries to collect data from them that allows us to better serve them.

 ### 48. Smooth form creation process and salesforce integration

**What do you like best about FormAssembly?**

You can do so many things with FormAssembly. Our favourites tools are the Salesforce connectors and Stripe connector. Formulas and advanced form logic allow us to use forms for so many different use cases. The support desk are always helpful and come up with useful tips and tools when we get stuck.

**What do you dislike about FormAssembly?**

I can't think of anything I dislike about FormAssembly - if there are things we haven't mastered yet it's got more to do with our level of expertise in all the features than any lack of functionality in the system.

**What problems is FormAssembly solving and how is that benefiting you?**

We use FormAssembly to manage virtually all of our data capture into our Salesforce CRM. This consists of forms to capture customer requirements, leads, bookings, applications, etc as well as interaction with customers and beneficiaries during various contact points in their journeys. We also use FormAssembly to conduct large surveys within our community.

 ### 50. FormAssembly Review

**What do you like best about FormAssembly?**

What I like most about FormAssembly is it's customization aspects and how it is able to integrate seamlessly with other systems.

**What do you dislike about FormAssembly?**

The only thing I could complain about this system is when human error comes into play and I can't build out a validation rule.

**What problems is FormAssembly solving and how is that benefiting you?**

FormAssembly is providing us with a way to administer forms while also collecting the information the forms provide and automatically transferring it to our CRM platform.
