Instead of typing every note from scratch, clinicians click through structured clinical findings — 12 mental status exam domains, a guided Subjective builder, interventions, and structured risk documentation aligned to the Columbia Protocol — and the app assembles a complete, professionally worded SOAP note. Eight standardized screeners (including the PHQ-9 and GAD-7) score themselves automatically, and four more accept scores you enter. Every sentence in a generated note traces to something the clinician actually selected or typed — there is no AI generation and no invented content. The app is 100% offline: no account, no cloud, no data collection, and it stores nothing on disk, so there is no PHI exposure to manage. Clinicians can verify this themselves with a firewall test.
