Cognivolt is an AI clinical scribe, clinical decision support tool and EMR in one product. It records or ingests a consultation and generates a structured note (SOAP, DAP, BIRP, History, or a custom template) in real time, along with a mental state examination and a structured risk assessment. Nothing downstream of a diagnosis — ICD-10/CPT coding, clinical guideline references, patient psychoeducation content — is generated until the clinician confirms the diagnosis themselves. Allergy and drug-interaction checks come from fixed reference tables, not a model's opinion. Beyond the note, Cognivolt includes a full EMR: searchable longitudinal patient charts, scheduling, an audit log, referral letters and discharge summaries, and FHIR export. Built for 17+ specialties — psychiatry, primary care, cardiology, neurology, dermatology and more — with specialty-aware note structure for each. Supports telemedicine and in-person sessions, and can document a non-English consultation into an English note. Built on HIPAA-eligible infrastructure with a Business Associate Agreement accepted at signup, end-to-end encryption, and no session audio retained after a note is generated.
