A web platform for the continuous telemonitoring of at-risk patients. It integrates multi-parameter monitors via HL7 and the organisation's own CRM, providing real-time alerts to help anticipate clinical complications. It improves patient safety and reduces adverse events by making vital data available in real time on any device, automating MEWS calculation, and reducing manual tasks for the clinical team. The platform allows for: - Remote, real-time monitoring; - Continuous monitoring of patients' vital signs in inpatient, emergency, outpatient and home environments, with transmission (and retransmission to television channels, for example) of patient data in real time. - Visualization and interaction; - Intuitive interface that enables centralized data visualization, with personalized dashboards and interactive features, such as the dynamic arrangement of monitors and direct access to details of clinical episodes. - Alarms and notifications; Configuration of personalized alarms based on physiological parameters (e.g. drop in oxygen levels or change in heart rate) or integration into the automatic calculation of the MEWS (Modified Early Warning Score), contributing to the early detection of clinical worsening. In the near future, there will be the inclusion of predictive analysis tools based on artificial intelligence, with a view to developing algorithms that anticipate diagnoses and interventions.
