It turns the record into corrective actions with an owner and a due date, keeps reminding you until each one is closed and its effectiveness checked, and builds the analysis and the report out of the records you already wrote. RECORD IT ON THE SPOT - Three record types: accident, near miss and observation (unsafe condition or unsafe act) - Photos and video, the location and unit, a short account and who reported it - Injured people with names masked by default, witnesses, lost days and damage - A stop-work register: jobs halted as unsafe and the approvals that let each one restart FIND THE CAUSE 5 Whys and a fishbone diagram: people, machine, method, material, environment and management.
