## Formstack Forms Reviews
  ### 1. Clunky, expensive, not worth your time

**Rating:** 0.0/5.0 stars

**Reviewed by:** Verified User in Wholesale | Small-Business (50 or fewer emp.)

**Reviewed Date:** April 09, 2026

**What do you like best about Formstack Forms?**

I liked that it solved a problem we had at the time and got rid of the paper form we were using and it integrated into Google Sheets.

**What do you dislike about Formstack Forms?**

Where do I even start? It is clunky, the design options are terrible, and unless you are a software developer, there is no way to make it look better. The integrations would constantly fail.

**What problems is Formstack Forms solving and how is that benefiting you?**

It is not solving any problems right now. It is causing problems. I just haven't had time to move our last workflow to Jotform.

  ### 2. Sales Ops

**Rating:** 0.0/5.0 stars

**Reviewed by:** Verified User in Information Technology and Services | Mid-Market (51-1000 emp.)

**Reviewed Date:** January 15, 2026

**What do you like best about Formstack Forms?**

It’s good for basic document generation and online forms.

**What do you dislike about Formstack Forms?**

The support has been horrible and not helpful at all. Sales overpromised on the product’s capabilities and features, and it hasn’t lived up to what was presented. Overall, I’m very disappointed with both the product and the service.

**What problems is Formstack Forms solving and how is that benefiting you?**

Data for document generation.

  ### 3. HORRIBLE and it keeps getting worse

**Rating:** 0.0/5.0 stars

**Reviewed by:** Verified User in Pharmaceuticals | Small-Business (50 or fewer emp.)

**Reviewed Date:** November 19, 2024

**What do you like best about Formstack Forms?**

HIPAA compliance is a plus. Mutliple users with different access points.

**What do you dislike about Formstack Forms?**

HORRIBLE across the board. To start... Absolutely criminal pricing. Second... Next to impossible to cancel...HUGE waste of time, there's no cancel button. I've been trying to cancel for 2 months... and in the meantime they have been billing me for those two months. And also they have just let me know that apparently I am on a yearly contract?? And I can't cancel for 8 months? After I spend another $4k+ with them?? I am so beyond disappointed. I have been a customer for 5+ years. It's an insanely expensive, subpar service. I am so beyond my limit with Formstack. On top of all of this, I have reached out when our form logic was just NOT working (REALLY bad look for us for our clients) and they gaslit me and told me it was working after it had been fixed. SO why when I did absolutely nothing did my form not work and now it all of sudden works??? Hm...

Go with another form provider. My business needs HIPAA compliant forms and there are way better alternatives out there for a FRACTION of the price. Like less than what I am paying PER MONTH at Formstack for an entire year kind of fraction of the price.

I would give 0 stars if I could. I've been getting ripped off for years and am mad and feel taken advantage of and now they are making it my problem. I could have saved $10k if i switched to another provider earlier. I wouldn't be so mad because I do take responsibility for my part in this... but it's also taken me HOURS to attempt to leave the service (they still haven't cancelled my account and each month that goes on they keep charging me!!!!), another 20 hours or so to rebuild my forms on another platform. I am beyond unhappy with them and FULLY recommend you go with a different form company.

**What problems is Formstack Forms solving and how is that benefiting you?**

I need forms to prequalify patients for clinical studies and formstack offered me a way to do that

  ### 4. Good product with very difficult cancellation policies.

**Rating:** 0.0/5.0 stars

**Reviewed by:** Mihai B. | Mid-Market (51-1000 emp.)

**Reviewed Date:** March 13, 2025

**What do you like best about Formstack Forms?**

It has been a good product, especially after the workflow integrations.

**What do you dislike about Formstack Forms?**

After more than 10 years of usage and continued promises, language and translation options were added. We moved away and have been trying to cancel for a couple of months but could never achieve it no matter what I tried. Got into a new billing cycle, and nobody cares to help but just point to the SLA.
Never gain and no more recommendations FS.

**What problems is Formstack Forms solving and how is that benefiting you?**

Forms for admissions and other processes.

  ### 5. Bad experience with Formstack

**Rating:** 0.0/5.0 stars

**Reviewed by:** Verified User in Leisure, Travel & Tourism | Mid-Market (51-1000 emp.)

**Reviewed Date:** October 12, 2023

**What do you like best about Formstack Forms?**

Nothing, for real. Others form builder provide the same features with more reasonable price and expected result, while Formstack is expensive and giving me a bad experience on it.

**What do you dislike about Formstack Forms?**

Had a bad day with feature disable dates past on input date. What I expected is disable past dates so user can only choose the incoming dates, but what Formstack did is disable FUTURE dates. And why the hell that I need to select a specific date to disable past dates, but not do like other form builder allow us to do like inputing `Today` keyword in the field to disable the past dates including today? Who will be happy to pay $429 annually but got this super lousy service?

**What problems is Formstack Forms solving and how is that benefiting you?**

Please provide the features that is convenient for users, and not misleading the users with the label

  ### 6. Probably works for simple usage, but didn't do what I needed

**Rating:** 0.5/5.0 stars

**Reviewed by:** David B. | Mid-Market (51-1000 emp.)

**Reviewed Date:** May 30, 2023

**What do you like best about Formstack Forms?**

Can build forms and connect with many platforms. Forms are pretty much industry standard; there are a ton of competitors that do the same work.

**What do you dislike about Formstack Forms?**

I didn't enjoy working with their support team after the fact. I Created an account and tried to a build with connections to Adobe Sign and FirstData payment process that should have been possible, but wasn't with upgrading and signing up for much more expensive tools. (4 - 5 times more per year).

**What problems is Formstack Forms solving and how is that benefiting you?**

Formstack was supposed to enable me to build a form that dynamically generates a contract and processes a deposit by connecting Adobe Sign and FirstData. The benefits: seamless contracts and automatic depositor management.

  ### 7. It was good for its time, but other solutions (like TypeForm) are better now

**Rating:** 0.0/5.0 stars

**Reviewed by:** Chris M. | CTO, Small-Business (50 or fewer emp.)

**Reviewed Date:** January 17, 2023

**What do you like best about Formstack Forms?**

Integration with other services (via Webhooks, emails, plugins, etc) is really great! It's also easy for non-devs to use, and easy to integrate into existing web-applications (such as our SPA app using Angular). The approval workflows also let us allow non-devs to approve or reject requests for common actions requested by customers.

**What do you dislike about Formstack Forms?**

The end-user experience isn't so great (for users submitting the forms). Mobile support could be better, and the reporting system is quite lacking. Solutions such as TypeForm are far superior experience for the end users.

Pricing is also a little high for the features it offers (which is essentially the same as just HTML forms support). It also doesn't allow having a pool of reviewers where ANYONE is allowed to review, nor does it have a convinient slack connection for approval workflows.

**What problems is Formstack Forms solving and how is that benefiting you?**

Formstack allows non-technical users to create and edit forms. It also lets us have an approval workflow that can allow certain internal staff to review, and approve, submissions form customers BEFORE triggering workflows internally that provision things (such as adding an email domain for a customer).

**Official Response from Scott Crabb:**

> Hello Chris, we love to earn 5 star reviews, but this feedback is even more valuable to our team.

We're sorry we didn't have the features you were looking for here. I'll make sure to pass your feedback to our product teams to consider for future improvements. If you have any other feedback you'd like to provide, just shoot us an email formstack@formstack.com

Thank you!

  ### 8. Very poor experience

**Rating:** 0.0/5.0 stars

**Reviewed by:** Verified User in Health, Wellness and Fitness | Small-Business (50 or fewer emp.)

**Reviewed Date:** June 07, 2023

**What do you like best about Formstack Forms?**

The consumer software product was great when we initially tried it out.

**What do you dislike about Formstack Forms?**

Deceptive sales practices where they forced us to buy a $50/month product for over $1,000 per month with a 1 year contract, threatening to cancel our retail product membership now that they knew about us. The reason provided was that we require HIPAA compliance and a BAA, even if we are not an American organization (we are located in Canada). We found out within a few days that this was not true since we do not provide any services in the US or to American citizens.

When tried to work with them, they were non-cooperative and started sending threatening court notices and letters. Their values state that "honesty is a value we cherish" and "Help others succeed". However this was not at all our experience with them and they have actively helped us become stressed out, insolvent and decide to close our business, and now 4 jobs are lost.

**What problems is Formstack Forms solving and how is that benefiting you?**

Formstack helped us set up some smart forms. The forms were relatively easy to set up initially but we could not do any of the enhanced configuration that was promised to us.

  ### 9. WORST CUSTOMER SUPPORT EXPERIENCE EVER - BEWARE

**Rating:** 0.5/5.0 stars

**Reviewed by:** Zech G. | Web Strategy, Mid-Market (51-1000 emp.)

**Reviewed Date:** January 25, 2022

**What do you like best about Formstack Forms?**

ability to integrate custom scripting into forms, however, the way that they implement forms is way too legacy and will make your website unable to pass Google Core Web Vitals. This is because they use document.write functions, which are not recommended by anyone ... but they are using this still. Google talks about "how to remove document.write" here: https://developer.chrome.com/blog/removing-document-write/

**What do you dislike about Formstack Forms?**

They say that this is a "codeless" form solution when you really need deep knowledge of CSS, scripting, etc to integrate properly. If you want a basic basic form, this is good, but the way they sell it is misleading.

Do not count on their customer support for anything. When I am asking them for help, but it's evident that they know less than a most about complex integrations, it's a waste of your time. I've reached out to their support no less than 10 times with no help whatsoever.

I've had an issue where the javascript in their forms is jumping the forms up on page. When embedded on their site or ours, the behaviour is the same, but they claimed it was not doing it on theirs, but just "not as much" as on my embed of the form. So we both agreed that it was jumping up, but they still claimed it was my problem to deal with and not theirs to assist me with... despite it being an issue in their javascript.

VERY DISAPPOINTED to be told over and over again that it wasn't their problem.

Here is their response again:

--------------------------

I regret that your experience with support has been less than ideal and appreciate you sharing your feedback with us.

In cases such as this where the issue is not reproducible on our end, not being reported by any other clients and the native form itself is working as expected - we can only conclude that there are some elements on your webpage that are causing it to jump like it is when embedded.

Once again, please consider reaching out to the professional services team to help you with this. Since we have no ownership over your website and are unable to reproduce the problem natively with the form, it is out of the scope of what our support and engineering teams can help troubleshoot.

I'm truly sorry for any inconvenience or delays caused as a result.

---------------------------

I've had 3 external javascript professionals tell me that it was the javascript on their side that was causing their forms to jump upon clicking the "next" button on their forms.

I've submitted tickets, got on a call where a "support person" told me that because the "name of the form" I had NAMED it was called "mobile only", that because of that, I was having problems? What kind of support is that!?? It had NOTHING to do with the form jumping up. It was simply what I had named it..

Sick of their support! sick of it. I've invested so much time and effort only to be called a liar at the end of this. So unprofessional.

**Recommendations to others considering Formstack Forms:**

If you need customization and support, it's really not for you. Their support team only knows how to use the forms, they don't know how to troubleshoot them, fix them or even send you in the right direction to fix them. If you are considering this solution and you have a budget that's any larger than 3K to build, it's better to go the custom route and hire a developer and have your forms developed and deployed locally. This form builder is a real thorn in the side of core web vitals. You will not pass with mobile as they are using legacy ways of displaying forms using "document.write" function which is inherently slow. On top of that, you cannot apply defer/async to these forms without them disappearing from your site.

**What problems is Formstack Forms solving and how is that benefiting you?**

adding forms to our website
FYI adding their forms is like dating the wicked witch. You'll get everything you need and nothing you want. You'll encounter some major roadblocks that only a developer will be able to overcome. For example: #1 If a user hits the "Submit" button two multiple times before the data is sent, the form will error out.

Embedding the form will require you to create custom javascript so that the forms don't bounce up to the top of your website. This was a real pain. Their customer support attempted to make me feel as if it were my problem and not theirs. After showing them this issue on a Wordpress site embed, a plain html site embed, they still said they couldn't reproduce the problem and therefore it was just my problem. They did not help us.

  ### 10. Great product, lackluster sales and support staff.

**Rating:** 0.5/5.0 stars

**Reviewed by:** Tom O. | Owner, Information Technology and Services, Small-Business (50 or fewer emp.)

**Reviewed Date:** January 08, 2021

**What do you like best about Formstack Forms?**

Solid technology that works well between al their products. Formstack is offering a suite of tools that other companies do not and it is exactly what companies actually need in my opinion.

**What do you dislike about Formstack Forms?**

Sales reps have trouble making scheduled meetings and responding in a timely manner to emails. Support staff are not very familiar with the products and my account was suspended and reopened a few times without notice or communication. I did get apologies for all of this, but there are continuing issues with sales and support. Looking elsewhere for this service.

**What problems is Formstack Forms solving and how is that benefiting you?**

Customer interaction and document signing. It works.

**Official Response from Lindsay Adams:**

> Hi Tom, we're sorry to hear about your experience. We're always looking for ways to improve our products and business processes, so your feedback is invaluable. We would love to have this addressed directly. If you would like to get in touch, please email support directly at support@formstack.com. Thank you.

  ### 11. Spotty submissions, slow support response - we bought a car with no engine.

**Rating:** 0.5/5.0 stars

**Reviewed by:** Nate N. | Managing Director, Mid-Market (51-1000 emp.)

**Reviewed Date:** January 15, 2019

**What do you like best about Formstack Forms?**

Helpful Salesforce integration saves times, very customizable branding options to designed multiple types of forms, fairly appealing interface.

**What do you dislike about Formstack Forms?**

We bought an engine with no car. Our users are submitting forms that are not recorded anywhere, and the support options are either incredibly slow, or, require you pay extra money every month.

**What problems is Formstack Forms solving and how is that benefiting you?**

Efficient nonprofit surveys and grant applications.

  ### 12. Forms are sometimes Great other times not so much

**Rating:** 1.0/5.0 stars

**Reviewed by:** Justin C. | EB-5 Immigration Investment Specialist, Investment Banking, Mid-Market (51-1000 emp.)

**Reviewed Date:** October 31, 2017

**What do you like best about Formstack Forms?**

If you are doing basic forms with a few fields and that is all you need I could see using this program.

**What do you dislike about Formstack Forms?**

Support is horrible. It can take a week to get anything done. You have to email back and forth and the rep doesn't spend any effort on figuring it out. It also is glitchy sometimes you have to leave the page and come back in order for it to allow you to keep working. It will also move fields around on you for no reason. The logic is also an issue on complex forms it cannot handle it well.

**Recommendations to others considering Formstack Forms:**

Don't expect to get help

**What problems is Formstack Forms solving and how is that benefiting you?**

I use them with Zapier to get the forms into Salesforce.
