Bring your existing intake packet, consent forms, surgical forms, or assessments, and our team can help configure the workflow. Adapt the packet to the patient. Conditional logic supports different visit types, body regions, and case types, including personal injury and workers’ compensation. Forms can include body diagrams, outcome assessments, consent language, guardian signatures, and provider countersigning. Patients can complete forms in English or Spanish and use their phone to capture insurance cards, ID, medications, and supplements. Work with or without an EMR. Collect and download completed PDFs for your existing records workflow, or connect a supported system. Open Dental integration is included with Pro. Pro + EMR includes one supported EMR integration; availability and capabilities are confirmed for your system and practice setup. Built for specialty workflows. EasyDocForms supports orthopedics, chiropractic, physical therapy, neurosurgery, pain management, pediatrics, podiatry, dermatology, functional medicine, and neuropsychology practices. Choose the plan that fits. Forms is $29/month for unlimited basic intake forms. Pro is $79/month for complex forms and Open Dental. Pro + EMR is $129/month for another supported EMR integration. Both upper plans include two doctors and unlimited staff, with each additional full-time provider at $20/month. A signed BAA is included with every paid plan.
