In terms of customizing the embed form, it does allow you to add your own custom fields, like the ones you mention. If you need help with that, reach out to us in support@donate.ly and we'll be happy to provide a snippet.
In addition to that, we're always evolving the base functionality. We have historically always had an 'On Behalf Of' field, but if a similar 'In Honor Of' field is something that many users will find useful - we're happy to add that to the roadmap.
