CareApps engage patients with: • Reminders to take vitals, complete assessment/symptom surveys, and take or discontinue medications. • Personalized and condition-specific content and encouragement as well as self-care guidance if/when appropriate. • Care team communication via chat, virtual visits, and one-click calls. Design Studio- Our user-friendly drag & drop Design Studio transforms clinical protocols into remote care programs with a few clicks. Branching logic enables survey responses and vitals measurements to trigger automated responses, patient-specific follow-up actions, and care team notifications. • Choose from validated surveys and questionnaires or use your own. • Determine which protocols to automate and how/when to receive notifications, adjust as needed. Care Team Dashboard- Provides care teams with customized views of patient data, and notifications for follow-up enabling clinicians to focus their time on patients they want to monitor. • Support for workflow automation and recommendation for proposed actions. • A multi-channel messaging system between providers and between patients and providers. • View data and trends by individual patients, cohorts, or entire populations.
