Clive is a browser-based tool that helps workplace incident investigators complete a 5 Whys root cause analysis and produce a report in Word. The investigator enters the incident details and uploads the evidence they have, such as witness statements, photos and records. Clive reads the evidence, then asks the investigator follow-up questions to fill gaps and resolve conflicting accounts. Once there is enough information, it drafts the analysis. The report includes: Incident summary and problem statement Five Whys causal chain, with multiple chains where the evidence shows more than one cause Root causes and contributing factors Risk rating for both the actual outcome and the maximum reasonable outcome, using a 5×5 matrix that can be set up to match the organisation's own Corrective actions, each checked against the hierarchy of controls Alignment with ISO 45001:2018 incident investigation requirements Sign-off section The investigator reviews the draft and can ask Clive to rework any part of it. The final report exports to Word for editing and filing. An investigation can be saved to a file at any stage and passed to a colleague, who opens it and continues from the same point. Clive works each causal chain through all five levels, from the immediate cause to the systemic condition behind it. If the evidence doesn't support a conclusion, it reports the root cause as undetermined rather than guessing. The investigator remains responsible for the findings. Clive runs in a web browser, with nothing to install. Each organisation has its own login, sites and employee assistance program details. It is built for contracting, mining, construction and industrial organisations, and is developed by Magnus Lion.. It works alongside Shannon, Magnus Lion's tool for gathering post-incident statements.
