Every insurer, case manager, solicitor and medico-legal company sends its own report form, and none of them match. Clinicians end up retyping the same notes into each one, often in the evening. ClinForms fills the referrer's own Word or PDF form, in its original layout, from your clinic's signed notes. Upload a referrer's form once, exactly as it arrived. ClinForms maps its questions and where each answer goes on the page. Your team checks the mapping once and it is reused for every patient. Then pick the patient, choose the form, and the answers are filled: facts from the record, narrative answers drafted from the whole episode of signed notes. Every answer shows the note it came from. Anything not in the notes, such as a prognosis, is flagged as a gap and never guessed. Opinions only ever come from the clinician. Nothing is issued until the treating physiotherapist has reviewed, completed and signed it. Then download the completed form in the referrer's own layout, or copy ready answers into an insurer portal. ClinForms works alongside the popular clinic systems using your notes export or PDFs, so you keep the system you have. It is also built into ClinDesk practice software as the Reports add-on. Priced by the number of reports each month, from up to 25 reports a month, covering all sites and all staff.
