What is this product? Patients can now receive personalized care from the comfort of their own home. From our remote care management solutions, Chronic Care Management (CCM), Behavioral Health Integration (BHI) to our first-of-its-kind artificial intelligent, cognitive assessment care plan builder, MyPLAN. We are working every day to advance the standard of patient care. At Advanta, we have a holistic approach and bring the human touch to patient care in between visits. This includes improving the patients access to care through a dedicated Advanta clinical care manager. Patients have access to their dedicated care manager as often as needed by calls and text. A support system is built to help the patient achieve their health goals, quickly receive medication refills help them with medication management, diet & nutrition relevant to their chronic conditions, understanding their social determinants of health, scheduling appointments with their primary physician and specialists, social support, disease education & symptom awareness, behavior modification and more. This results in improved workflows for clinics reducing administrative burden allowing them to focus on patient care. We communicate all clinically relevant information back with the clinic via an internal chat and documenting on the patient's chart in the clinic's EHR. We able to offload over 50%+ of clinical escalations of Medicare/Medicare Advantage patients to the Advanta team. Patients see an increase in monthly patient care of 41 minutes when enrolled in CCM and 60 minutes when enrolled in CCM & BHI. This results in an increase of $250,000 in NET profit to the clinic for every 500 patients enrolled.
