We use 123 Form builder for tons of our forms which collect data every day. It is easy to use, highly functional and configurable, and never fails.
We especially like that it has a built-in publishing component that can be used to publish in multiple ways. Another great feature is the way it works to build reply emails automatically.

The links to your forms can be customized by replacing our domain (123formbuilder.com) with your own, by using the domain alias feature. You can read more about this feature here: https://www.123formbuilder.com/docs/domain-aliasing/. The only element that cannot be removed is the form ID, which is necessary in order to identify the form.
