Sign into Hitachi ID Identity Manager web portal, select ’View and update profile’ under ’My profile’ and select a request form from the list on the bottom of the page. Each request form steps you through a ’Wizard’ with a sequence of simple panels and ’Previous’/’Next’ navigation. Read more here: https://hitachi-id.com
