Puede agregar imágenes de encabezado y si está utilizando demasiadas preguntas, le sugeriría anidarlas, lo que podría verse bien. No hay muchas funciones que embellezcan su formulario, sin embargo, si es usuario de Google Sites, puede agregarlo allí y embellecer la página en su lugar.
