At the moment, you can integrate your forms with Gmail and Slack via Zapier. We're working on direct integration with Slack, it will be launched soon.

You can find our templates here: https://forms.app/en/templates
It's very simple to use them. Find the one you're looking for, open it, click on the "Use Template" button when you're logged in and you will have that template on your MyForms panel. After that, you can customize it according to your needs.
