No, I have created several surveys and as long as I share them publicly then whomever opens  it can respond and fill in the forms. This is very useful for crowd sourcing citizen science information that a lot of people can access and use.

No. Not necessarily. If you share your surveys publicly, anyone can submit data using the Survey123 web and mobile apps. An ArcGIS user is only needed if you want  to keep your survey private to your organization.
