On SignRequest's template FAQ, https://help.signrequest.com/hc/en-us/articles/115002895645-How-to-create-templates-

go to number - 2. Templates with tags in a base document
This explains how to add tags to a document, and it will automatically create fields on upload.

In documents I have used to do this I create the tags is black text then convert it to white text so it will not be seen in the uploaded document, but it is still recognized by the software and creates the fields.

one thing to note, signer 0 should always be you, signer 1, 2, etc. are other people when creating tags.
